In-Network providers can also be found on your provider’s website at www.myuhc.com under “Find a Provider” and then you can search by line of coverage, location, provider/facility name, or search by specialty.

Did You Know? 

  • Preventive Services are covered at 100% In-Network and copays & deductibles do not apply.
  • You pay less out of pocket if you receive care from an In-Network provider.  
  • You do not need a referral to see a Specialist.

Pre-treatment Estimate

  • If your dental care is extensive and you want to plan ahead for the cost, you can ask your dentist to submit a pre-treatment estimate. While it is not a guarantee of payment, a pre-treatment estimate can help you predict your out-of-pocket costs.

  • 24/7 dental care for dental emergencies when an in-person visit isn’t an option. Available for active dental members*.

How do I find an In-Network Provider?


Under the UNUM Vision plan, you may use the eye care professional of your choice. However, when you visit a participating EyeMed provider, you receive higher levels of coverage. If you choose to receive services from an out-of-network provider, you will be required to pay that provider at the time of service and submit a claim form for reimbursement.


Members have the freedom to choose any provider from EyeMed’s Insight Network. Our network offers the right mix of independent, national retail and regional retail providers like Lens Crafters, Pearle Vision, Target Optical and many more. Members can also purchase glasses and contact lenses online at Glasses.com and ContactsDirect.com.

LIFE AND AD&D


DMCA provides you with Basic Life/AD&D coverage at no cost to you!.

 

Basic Life/AD&D

Employees working 30 or more hours per week are eligible for $30,000 coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.

 

Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

DISABILITY


Long Term Disability (LTD)

Disability benefits are designed to replace a portion of your income if you are unable to work due to an accident or sickness. Benefits begin after a 90-day elimination period are are equal to 60% of covered monthly earnings up to $6,000 for full-time employees. The benefit that is paid to you taxable.
