Phoenix Residence is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Eligibility
Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Phoenix Residence.
In addition, the following family members are eligible for certain benefits as well:
- Your legal spouse. This does not include a common law spouse or an ex-spouse.
- Your (or your spouse's) biological child, stepchild, a child for whom legal guardianship has been awarded, and who are less than 26 years of age.
- Unmarried dependent child who is physically or mentally disabled and is dependent on you.
New Hires
Enrolling in Benefits will be a 2 step process:
All benefits except medical will be done by using a platform called Employee Navigator.
For medical enrollment please take the following steps:
- Please see the ICHRA notice for The Phoenix Residence’s ICHRA contribution as well as additional important details regarding this offering.
- Sign Up. Visit www.gravie.com to sign up for your Gravie account.
- Enroll in Your HRA. The first step is to complete enrollment in your HRA. You will select the Enrollment tier that corresponds with your enrollment tier for your individual health plan. For example, select “Employee + Family” if you intend to enroll your spouse and children in your health plan as well.
- Shop for Your Health Plan and Enroll. Compare options to find the plan that best fits your needs. Sort by monthly cost, deductible, and more. We’ve made it easy to check if your doctors and prescriptions are covered by your plan! Once you select a plan, continue through the checkout process to complete enrollment.
- Pre-Tax vs. Post Tax Payroll Deductions. While shopping please pay attention to the tax eligibility for each plan. When shopping through Gravie’s website, the non-exchange plans will be noted as pre-tax.
- Take advantage of Gravie Care. We know shopping for health insurance can be confusing, but we promise we’ve made it easy. The Gravie Care Team is availableto help answer all of your insurance questions.

The benefit choices you make during your initial enrollment or annual open enrollment remain in effect from 12/1/2026-12/31/2026.
You can, however, modify your elections under certain circumstances called "Qualifying Life Events."
QUALIFYING LIFE EVENTS
You can change your benefit election during the year if you have a qualifying life event. Qualifying Life Events include, but are not limited to:
- Marriage, divorce or legal separation
- Birth or adoption of a child
- Change in child’s dependent status
- Death of a spouse, child or other qualified dependent
- Change in employment status or a change in coverage under another employer-sponsored plan
If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.

Gravie will continue to administer your medical and prescription drug benefits for the 1/1/2026-12/31/2026 year.
Introducing ICHRA
What is ICHRA? The Individual Coverage Health Reimbursement Arrangement is a group benefit plan provided by your employer to contribute funds that can be applied toward eligible medical expenses on a pre-tax basis. Eligible ICHRA expenses with Gravie are:
- Premiums for qualified health plans purchased through the individual market
- Premiums for Medicare Part A, B, or C

MetLife's plan provides comprehensive coverage to help you and your family maintain good dental health.
The MyBenefits web site is a quick and easy way for you to get the information you need about your MetLife benefits — all in one place. Log in at metlife.com/mybenefits to see how we’ve taken personalization and integration to a new level.
Get more information on your MetLife benefits, where you can link to detailed coverage information and can perform tasks, such as:
- Dental Plans — Easily find a participating dentist or view your benefits, copay or coinsurance amount, and claims online. Plus, you will have access to our extensive Oral Health Library to research important dental topics.
- Dental ID cards are available online for you to download and print at your convenience. Cards contain your name, employer’s name and group number. Also included are MetLife’s claims submission address, website address, customer service telephone number and a service number for International Dental Travel Assistance.
Be aware if you go out-of-network, your costs will be higher. To find out if your provider is in-network, please visit MetLife's website below.

DID YOU KNOW?

Phoenix Residence offers vision coverage through EyeMed to help pay for prescription glasses and contact lenses.
You receive a higher level of benefits when you see a provider in the EyeMed Network. To find out if your provider is in-network, please visit the EyeMed website below.
Voluntary Life
Phoenix Residence provides you with Voluntary Life coverage through Reliance Standard.
Voluntary Life
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
