Medical Benefits
Medical: Anthem BC BS
You have three plans to choose from:
- PPO plan 80% / 50% - deductible $4000 / $8,000
- PPO plan 80% / 70% - deductible $1,000/ $2,000
- HSA plan 100% / 70% - deductible $3,000 / $6,000
Please Note-
There is a $75.00 weekly surcharge if you spouse has access to insurance through their employer; however, chooses to enroll in mDesign’s plan.
There is a $5.00 weekly surcharge for tobacco users.
The plan requires an annual wellness check. The wellness check saves you a $12.00 weekly surcharge.

Dental Benefits

- Split Value - you have the choice to visit any dentist, but you will pay less out of pocket when you access a PPO dentist.
