Employee Benefits Effective through 12/31/2023

WELCOME

Helping you and your family reach and maintain good health is very important to Nitro Swimming. Nitro Swimming is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind.

We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Nitro Swimming. When you enroll in the benefits program, you may also cover your eligible dependents.

MEDICAL

There are three medical plan options through Humana to choose from.

One plan is a High Deductible HSA compatible plan, the other two are PPO copay plans with lower Deductibles and Co-pays.

With the Humana plans, you and your dependents can visit any provider, in network or out, without a referral. However, when you stay in network, Humana will

handle the claims and offers lower, contracted rates.

 

HSA – This is a high deductible HSA compatible plan. It has in and out of network benefits. You must pay the contracted rate with the provider and meet your deductible before copays or coinsurance applies.

~~ NOTE - If electing the HDHP/HSA plan you will need to contribute at least $1.00 per pay period to the HSA account when you complete your enrollment in the EASE portal.

Your employer also makes a monthly contribution to the HSA. Account. ~~


PPO – This is a regular PPO plan. It has in and out of network benefits. This plan has a lower deductible and copays for the little things such as doctor’s visits and prescriptions.

 

You can search for a provider below. The PPO and HSA plans are both in the Humana National POS – OpenAccess Network.

DID YOU KNOW?

  • Eye exams can help detect serious eye and general health conditions sooner: high blood pressure, diabetes, heart disease, high cholesterol
  • Babies should receive their first professional eye exam at 6 months
  • 80% of learning in the first 12 years comes through the eyes


This vision plan offered through Humana Vision will help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in the Humana EyeMed Network. Review coverage details in your Benefit Guide.

EMPLOYER PAID LIFE

Full Time Employees working 30 or more hours per week are eligible for $35,000 coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.

 

Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

ENROLLMENT

How To Enroll In Benefits

The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.

 You can, however, modify your elections under certain circumstances, called "Qualifying Events". These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.

IF YOU EXPERIENCE A QUALIFYING EVENT...

You must reach out to Jon Gwin to unlock your ability to make Qualifying Event changes through the EASE enrollment portal.

You will not be able to make changes through the portal until Jon unlocks your portal for the event.

 

EMPLOYEE ELIGIBILITY

You are eligible to participate if you are full-time and work a minimum of 30 hours per week. Your coverage will be effective 1st of the month following 60 days.

 

ENROLLMENT

New Hire Enrollment - Nitro Swimming will send you an email with a link inviting you to log in to the EASE Portal to set up a password and complete your New Hire Enrollment.

ALL Eligible Employees are REQUIRD to complete the enrollment process even if you are waiving coverage.

No exceptions.

All team members have access to our Ease online benefits enrollment platform 24/7 where you have the ability to enroll, select or change your benefits online during the annual open enrollment period, new hire orientation, and for qualifying events.

YOUR EASE PORTAL IS LOCKED FOR QUALIFYING EVENT CHANGES.

PLEASE REACH OUT TO JON GWIN TO UNLOCK YOUR PORTAL WHEN YOU EXPERIENCE A QUALIFYING EVENT.

ONCE JON UNLOCKS YOUR PORTAL ACCESS, YOU WILL HAVE 30 DAYS FROM THE DATE OF THE EVENT TO MAKE YOUR CHANGE.
