Welcome

Helping you and your family reach and maintain good health is very important to Legacy Lockers, LLC. Legacy Lockers, LLC is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Legacy Lockers, LLC. When you enroll in the benefits program, you may also cover your eligible dependents.



Enrollment

How To Enroll In Benefits


The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.

 

You can, however, modify your elections under certain circumstances, called "Qualifying Events". These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

 

EMPLOYEE ELIGIBILITY

You are eligible to participate if you are full-time and work a minimum of 30 hours per week. Your coverage will be effective 1st of the month following 60 days from your date of hire.

MEDICAL

We have three medical plan options through United HealthCare. One option is a PPO plan, one is an EPO plan (no out of network services), and one is an HSA compatible plan. See summaries and links below for more information.


Both the PPO and EPO plans have a $0 primary care copay for kids under 19.


PPO – This plan has in and out of network benefits. It has copays for the little things such as doctor’s visits and prescriptions.

EPO -This plan has in network benefits only and no out of network benefits. It has copays for the little things like doctor’s visits and prescriptions.

HSA – This is a high deductible HSA compatible plan. It has in and out of network benefits, but you must pay the contracted rate with the provider and meet your deductible before copays or coinsurance applies.

 

All plans include telemedicine through Healthiest You. This is telemedicine 24 hours a day, 7 days a week.


NOTE:

CVS in no longer in network with new UnitedHealthCare Plans.


You can search for a provider below. The PPO and HSA plan use the Choice Plus network. The EPO plan uses the Choice network. There is almost a 100% overlap of these networks.

DENTAL

Dental Select Dental Plan

You have two dental options to choose from. Both of these plans provide comprehensive coverage to help you and your family maintain good dental health. Providers on both plans are part of the Dental Select Platinum Co-Insurance network, or you may see an out-of-network dentist of your choice. Both of these plans pay out of network at a high reimbursement rate (90th R&C) but you could save money by staying in network.

COLONIAL

With most plans, you receive benefits regardless of any insurance you may have with other companies.
