WELCOME to Geek Powered Studios Benefits OPEN ENROLLMENT

Helping you and your family reach and maintain good health is very important to us.

Geek Powered Studios is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind.

We review our benefit programs each year to ensure we consider the best combination of benefit coverage,

network access and affordability for employees.






ENROLLMENT

OPEN ENROLLMENT will begin Friday, 9/9 and close Tuesday, 9/20.

Elections made during the Open Enrollment period will be effective October 1, 2022.

You will complete your annual Open Enrollment through the EASE portal. (Link below)

ALL Employees are required to log in to EASE and complete the enrollment even if you are waiving coverage.


All team members have access to our online benefits enrollment platform 24/7 where you have the ability to enroll, select or change your benefits online during the annual open enrollment period, new hire orientation, and for qualifying events.

  • Accessible 24/7;
  • View all benefit plan options and your elections;
  • View important carrier forms and links;
  • Report a qualifying life event; and
  • Make changes to beneficiary designations and more.


Choose your benefits carefully.

The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.

 You can, however, modify your elections under certain circumstances, called "Qualifying Events".

These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan.

If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.

 

EMPLOYEE ELIGIBILITY

You are eligible to participate if you are full-time and work a minimum of 30 hours per week.

Open Enrollment elections will begin on October 1, 2022.

New Hire coverage will be effective the 1st of the month following 60 days from your date of hire.

Geek Powered Studios offers 4 Medical plan options to choose from.


PPO | In-Network & Out-of-Network Benefits Available

The PPO option offers the freedom to see any provider when you need care.

•When you use providers from within the PPO network, you receive benefits at the discounted network cost.

•Most expenses, such as office visits, emergency room and prescription drugs are covered by a copay. Other expenses are subject to a deductible and coinsurance.

HMO| In-Network Services Only

An HMO gives you access to doctors and hospitals within the HMO network.

Unlike PPO plans, care under an HMO plan is covered only if you see a provider within that HMO’s network. 

  • All employees and dependents selecting the HMO option are required to select a PCP (Primary Care Physician)

in the Blue Cross Blue Shield HMO Network.

  • Seeing a Specialist requires a referral by your designated PCP.

Dental Coverage is provided at no cost to ALL eligible employees.

You may choose to add your qualified dependents through payroll deductions


Dental coverage through Principal Financial Group provides comprehensive coverage to help you and your family maintain good dental health. You may select the dentist of your choice; however, when you visit a participating in-network dentist you could have lower out-of-pocket costs, no balance billing, and claims will be submitted by your dentist on your behalf.

VISION

PRINCIPAL

Vision coverage is provided at no cost to ALL eligible employees.

You may choose to add your qualified dependents through payroll deductions.


This plan assists with copayments for eye exams, prescription glasses and contact lenses.

You receive a higher level of benefits when you see a provider in the VSP Network.

  • Babies should receive their first professional eye exam at 6 months

LIFE Insurance | Voluntary Life Insurance

PRINCIPAL

Geek Powered Studios provides all eligible employees with Basic Life/AD&D coverage at no cost.


Basic Life/AD&D

Employees working 30 or more hours per week are eligible for $30,000 Basic Life coverage payable to your designated beneficiary in the event of your death.

An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is

the result of an accident.

 

Who's Your Beneficiary?

Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event,

such as a divorce or birth of a child. Your Life Beneficiary information can be updated at anytime through the EASE Portal.


Supplemental Life

In addition to your employer provided Basic Life insurance coverage, you have the opportunity to enroll in voluntary/optional/supplemental Life insurance

coverage. Coverage is also available for your eligible dependents, however, It is required that you elect coverage for yourself in order to elect

coverage for your dependents. 


*Guaranteed Issue (GI) and Evidence of Insurability (EOI)

When you are first eligible (at hire) for Voluntary Life and AD&D, you may purchase up to the Guaranteed Issue amount (GI) for yourself and your spouse without providing proof of

good health (EOI). Any amount elected over the GI will require EOI. If you elect optional life coverage and are required to complete an EOI, you will receive an email from Principal with a link to complete the Evidence of Insurability on-line. Elected coverage over the GI amount will not be installed until the EOI is completed and the carrier has approved it. In addition, your spouse will need to provide EOI to be eligible for coverage amounts over GI, or if coverage is requested at a later date.

Cost of Coverage

Premiums are based on age-rated tables and paid by the employee every pay period through a payroll deduction. These premiums are post-tax and benefits payable are tax-free.

Employee Coverage

Choose in $10,000 increments up to a maximum of $300,000 - GI Amt - $70,000

Spouse Coverage

Choose in $5,000 increments up to the lesser of 100% of the amount you elect for yourself or $100,000 - GI Amount $20,000

Dependent Coverage

$2,000

Do I have to take a health exam to get coverage?

If you and your dependents enroll in coverage at your initial eligibility date, you may apply for up to the Guaranteed Issue amounts without medical questions.


At Open Enrollment:

If you are currently enrolled, you can add an additional $10,000 or $20,000 in coverage for yourself with no EOI required. Every year during open enrollment, you can continue to increase your coverage up to the maximum benefit. If you have coverage for yourself, you can also add or increase coverage for your spouse and children with no health questions asked.

EX... Employee currently has $30k, they could increase to $40k or $50k at open enrollment without EOI.

Any employee who previously declined Supplement Life coverage and wants to add the coverage at Open Enrollment can purchase $10,000 or $20,000 in coverage for themselves with no health questions and up to 100% of their elected amount for their Spouse. (Spouse amt may not exceed Employee amt)

Vol. Accidental & Critical Illness

Principal

If something happened to you, would your family have the income they need?


Voluntary Accident insurance helps offset the unexpected medical expenses that can result from a covered accidental injury. This coverage pays a lump-sum benefit for injuries received from an accident. See plan summary for specific coverages. All active full-time employees working at least 30 hours per week are eligible.


Voluntary Critical illness insurance supplements your major medical coverage by providing a lump-sum benefit that you can use to pay the direct and

indirect costs related to a covered critical illness. All active full-time employees working at least 30 hours per week and their dependents are eligible to enroll in this coverage.
