We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Eligibility
Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Frontier Justice, LLC. When you enroll in the benefits program, you may also cover your eligible dependents.
Helpful Tips To Consider Before You Enroll
Do you plan to enroll an eligible dependent(s)?
If so, make sure to have their social security numbers and birthdates available. You cannot enroll your dependent(s) without this information.
Have you recently been married/divorced or had a baby?
If so, remember to add or remove any dependent(s) and/or update your beneficiary designation.
Did any of your covered children reach their 26th birthday this year?
If so, they may no longer be eligible for benefits, unless they meet specific criteria.
The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.
You can, however, modify your elections under certain circumstances, called "Qualifying Events". These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.
EMPLOYEE ELIGIBILITY
You are eligible to participate if you are full-time and work a minimum of 30 hours per week. Your coverage will be effective 1st of the month following 30 days from your date of hire.

We are moving to United HealthCare Level Funded for medical.
You will have two medical plan options to choose from. These plans are similar to the plans that were offered last year.
Both plans are PPO options and have a $0 Primary Care copay for kids under 19.
Both PPO plans have in and out of network benefits. They offer copays for Doctor’s Visits, Urgent Care, and Prescriptions.
Please note – CVS is no longer an In Network provider for Prescriptions through any United Healthcare plan.
NEW! All members will have a $0 Telemedicine copay with HealthiestYou. HealthiestYou is available to all your dependents - even those not currently on your medical coverage.
Delta Dental
The Delta Dental plan provides comprehensive coverage to help you and your family maintain good dental health. Providers are part of the Delta Dental PPO and Premier dental networks, or you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your costs could be higher.
