How to Enroll for 2023 Benefits

ENROLLMENT INSTRUCTIONS:


1.Click on the BenXpress Tile and that will direct you to BenXpress

2.Enter your User ID: Employee ID Number

3.Password: Last four of social security number

4.Follow instructions and enroll in your benefits

5.Make sure to save your elections and print your confirmation statement.

6.Electronically update Employee Spousal Waiver Statement

7.As an option, you can download the BenXpress app and make open enrollment elections on your phone. Go to Apple app or Google play to download the app on your phone or tablet.

8.Employer Statement can be downloaded on BenXpress Resources tab or tear off last page of benefit Alert.

RX Benefits

Express Scripts (ESI) administers the prescription drug program through NFP’s Rx Solutions and a separate ID card must be used.

The specialty drug coverage includes a copay assistance program. Below are the tools and resources that are available to help you access the prescription drug information.



Registering with Express Scripts to create an online account:

•Go to Express-Scripts.com/activate

•Click on Register (upper right hand corner)

•Complete name, date of birth and email address

•Enter your social security number or member ID from Express Scripts

•Establish username and password

Your Express Scripts Account can help you with the following:

1.Under Prescriptions

•Refill Prescriptions by phone, online or mobile app

•History of prescriptions

•View Claims & Balances

•Price a medication

•Set up automatic refills

•Find a Pharmacy

2.Mobile App

•Use the Express Scripts mobile app by downloading from your mobile devices app store (Use the QR code on the Mobile App flyer to download the Express Scripts App)

Health Savings Account

NOTICE TO Employees Regarding HSA Bank

•UMR will automatically enroll you in HSA Bank if enrolled in the medical plan.

HSA Bank will send a Welcome Kit and Health Benefits Debit Cards. The debit card (two cards) will be used to pay for qualified expenses

•STEP 1: Employee signs in into their new HSA Bank account

•STEP 2: New User needs to be selected

•STEP 3: Use the credentials given in Welcome Kit to sign in

•STEP 4: HSA Bank will need you to designate a beneficiary

•STEP 5: If you wish to also contribute to the HSA, a Pretax Payroll Contribution from must be completed. This form will be linked to BenXpress

Once log-in has been completed, download the HSA Bank mobile app with the same credentials used for the initial sign-in.

Investment Options are available (must maintain $1,000 minimum balance) in your HSA Account: Devenir & TD Ameritrade.

HSA Bank Mobile App allows you to check balances and view transactions.

BASIC LIFE and AD&D INSURANCE

Life insurance is an important part of your financial security. Life insurance helps protect your family from financial risk and sudden loss of income in the event of your death. Accidental Death & Dismemberment (AD&D) insurance is equal to your Life benefit in the event of your death being a result of an accident and may also pay benefits for certain injuries sustained.

Cost of Coverage

Premiums are based on age-rated tables and paid by the employee every pay period through a payroll deduction. These premiums are post-tax and benefits payable are tax-free. The age-rated tables and premiums are programmed into the BenXpress online enrollment system. The amount of coverage for Life and AD&D must be the same amount.

Do I have to take a health exam to get coverage?

If NEWLY HIRED and enrolling within 30 days of eligibility, you may elect coverage up to the plan maximum; however, the first $125,000 will be allowed on a guarantee issued basis. Amounts elected above $125,000 will need evidence of insurability (EOI) with underwriting approval. Existing employees who wish to newly enroll or increase existing amounts, must complete evidence of insurability (EOI) submitted to underwriting review and approval.

LONG-TERM DISABILITY (LTD)

Serious illnesses or accidents can come out of nowhere. They can interrupt your life, and your ability to work for months – even years. 

Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.


Cost of Coverage

Ausco Products provides a company paid Long Term Disability benefit at no cost to you. You must be deemed to be disabled by a physician and unable to perform your job duties.

Elimination Period

This is the number of days that must pass between your first day of a covered disability & the day you can begin to receive your disability benefits.

Your elimination period is 180 days

Benefit Duration

The maximum number of weeks you can receive benefits while you are sick or disabled.

Payments will last for as long as you are disabled, or until you reach Retirement Age (age 65), whichever is sooner.

You must be sick or disabled for the duration of the elimination period before you can receive a benefit payment.

What's covered?

A variety of conditions and injuries. Typical claims would include cancer, back disorders, injuries and cardiovascular, joint disorders.

WHEN CAN I USE Telemedicine? 

When you have a minor illness or injury and:

–your doctor is not available;

–you become ill while traveling;

–when you are considering visiting a hospital emergency room for a non-emergency health condition.

*Your covered children between ages of 6 – 18 may also use Telemedicine when a parent or legal guardian is present.

1.Call 888.691.7867 and press 1 to speak to a doctor

2.A First Stop Health intake agent will ask a few brief questions

3.Within 5 – 10 minutes, a physician will call you back to review history, symptoms and make recommendations.

4.If a prescription is required, patients will be notified via email or text when the prescription has been sent to your preferred pharmacy. PLEASE HAVE YOUR PREFERRED PHARMACY’S PHONE NUMBER AVAILABLE.

Employees are automatically eligible for an Employee Assistance Program (EAP) and is provided at no cost through MetLife

 

EAP Overview

Provided through the LifeWorks EAP Program that provides consultation for relationship issues, stress, coping with change, grief and loss, family and marital issues, adjustment disorder, depression, anxiety, addiction and domestic violence

In-Person sessions that includes up to 5 face-to-face sessions per issue. There is no limit to the  number of issues per year.

Live, 24/7 access to a master’s level consultant. LifeWorks clinicians have a minimum of 3 years of clinical experience.

Assists in locating childcare and elder care services.

Includes referrals to a local attorney that provides a 30-minute consultation for free. A 25% discount applies for services beyond the initial consultation.

On-staff accredited financial consultants who can assist with credit management, budget analysis, home buying, mortgage/refinancing, retirement planning, basic estate planning.

Consumer and Convenience Research—provides consultation, research and referrals on consumer issues, home and appliance repair, pet services, transportation services, house/apartment sitting resources and purchasing large ticket items.
