WELCOME

This is your opportunity to learn about the Zeal Credit Union benefit plan offerings. Please review your 2022 Benefit Guide to see what plans are available with Zeal Credit Union. Once your enrollment window has closed, you will not have the opportunity to make changes to your elections until open enrollment, unless you have a Qualifying Life Event.



Once you have reviewed your benefit information and are ready to submit your elections, please visit www.ZealConnection.com. Your elections must be submitted within 14 days of employment and benefits will begin the first of the month following 60 days of employment.


Zeal Credit Union offers a $50 health incentive for waiving medical. Please submit proof of other insurance to human resources as soon as possible.

ENROLLMENT INSTRUCTIONS:

1. Go to www.ZealConnection.com

2. Enter your User ID: First Initial + Last Name (ex.: John Smith = JSmith)

3. Enter your Password: Last 4 of your SSN

4. Follow instructions and enroll in your benefits

5. Make sure to save your elections and print your confirmation statement.


The Benefit choices you make during your open enrollment remain in effect for the entire year.


You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.


To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:

  • Covered by a Qualified High Deductible Health Plan
  • You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
  • You cannot be covered by Medicare or Medicaid
  • You cannot be eligible to be claimed as a tax dependent
  • You have not received VA benefits in the past 90 days

Zeal Credit Union provides those employees who are covered by one of the BCBSM/BCN medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.


How Much Does it Cost?

  • HMO HBL 500 (Enhanced) : $20 copay
  • HMO HBL 1250 (Standard): $30 copay
  • HMO HSA 1400: 20% coinsurance after deductible
  • PPO HSA 2000: 80% after in-network deductible
  • Prescriptions: If prescribed during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in.

Zeal Credit Union offers vision coverage through National Vision Administrators (NVA) to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. Review your coverage details in your Benefit Guide. Zeal Credit Union pays 100% of the monthly premium for employees and their qualified dependents.

Zeal Credit Union provides you with financial protection options for life, accidental death & dismemberment, short term disability and long term disability with MetLife.


Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


  • Basic Life/AD&D Employer Provided Benefit - No Cost to You
  • 3x base annual earnings up to a maximum of $750,000


  • Voluntary Benefit - Costs are based on Age and Amount of Coverage
  • Employee: Choose in $10,000 increments up to the lesser of 6x your annual salary, or $500,000 - Guaranteed Issue: $100,000
  • Spouse and Child: Choose either a flat $5,000, or $10,000 in coverage - Guaranteed Issue: $10,000


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.


  • STD Employer Provided Benefit - No Cost to You
  • Cover 60% of your weekly earnings up to a maximum benefit of $3,500 per week (Benefits begin on the 1st day for Accident / 8th day for Illness)


Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. 


  • LTD Employer Provided Benefit - No Cost to You
  • Benefit is equal to 60% of your monthly earnings up to a maximum benefit of $6,000 per month

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.


New for 2022 - those who elect the Dependent Care FSA will now receive a debit card.


Contributions

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.


You may contribute as follows:

Health Care FSA

  • Up to $2,850 annually
  • All benefit eligible can participate
  • If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses


Dependent Care FSA

  • Up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns

PAID TIME OFF & HOLIDAYS

Paid Time Off: An employee annual allotment of PTO shall be determined by the length of service as of December 31st of the year prior to the year in which PTO is to be taken. New hire PTO amounts will be discussed at point of offer. Additionally, four hours of PTO are accrued each full month worked. Up to 40 hours of PTO not used in a calendar year may be rolled over or cashed out (must have 2 years seniority) at Credit Union discretion and with management approval.

401K

Employees are eligible for the following benefits after completion of 6 full months of employment:


Employees are offered a 401(k) plan through MCUE and CUNA Mutual. The Credit Union contributes 5% of their bi-weekly earnings into their 401(k) plan. In addition, employees are offered ability to make contributions into their 401(k). The Credit Union offers a dollar for dollar match on the first 3% of employee contributions. The 6-month waiting period will be waived for employees who were previous participants in the MCUE/CUNA Mutual retirement accounts.

  • Tuition Reimbursement- Employees are offered tuition reimbursement for undergraduate or graduate job-related courses. The costs of tuition and registration fees are reimbursed up to an annual maximum of $7,500 per calendar year ($3,250 for part-time employees).
  • Employee Assistance Plan (EAP) - All employees and their families have access to use the EAP services through MetLife and Ulliance. Representative who can assist you through work-life balance, stress management, grief, counseling, and much more. They will also help you find resources within your community.
  • Will Preparation & Financial Services - All employees have access to the financial coaching and will preparation services available through MetLife Advantages.
