We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. Yield Solutions Group remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to our associates. Highlights of the 2024 benefit plans include:

Eligibility & Enrollment
Who is Eligible for Coverage?
- All regular full-time employees working a minimum of 30-hours per week
- Your legal spouse or domestic partner
- Children up to age 26
- Children until they turn 26 regardless of student, marital, or employment status. This includes natural children, stepchildren, adopted (or placed for adoption), children of your domestic partner and children for whom you are legal guardian.
New Hires
Eligible employees are able to participate in the Yield Solutions Group benefit plans on the 1st day of the month following 30-days of full-time employment. You will receive benefit enrollment instructions with your new hire paperwork and will have 30-days to make your selections.
Online Benefits Access
Step 1: To log into Ease, open your web browser and type
enroll.benefitsconnect.net into the top tool bar.
Step 2: At your home screen, you will see a username and password welcome box.
EXAMPLE:
Joe Smithson
SSN: xxx xx 6789
Username: smithsj6789
Password: xxxxx6789
Step 3: Your username is the first six characters of your last name (if applicable), followed by the first letter of your first name, which is then followed by the last four digits of your Social Security Number.
Step 4: Your initial password is your Social Security Number (SSN). Please note, your SSN should contain no spaces or dashes.
Step 5: Click Sign In to enter Ease.
Step 6: Follow the prompts to complete your benefits enrollment.
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Medical & Rx
Cigna will continue to be your medical and prescription carrier. You have the choice between three plans for the 2024 plan year. The three plan options include two PPO Copay plans and a QHDHP plan. View the Providers & Networks for more information on how these plans networks differ from each other as well as buttons to locate participating providers.
If you are already enrolled in one of the Cigna medical plans today and have not already done so, please be sure to register through the Cigna member portal where you can access important information pertaining to your medical plans including participating providers, cost estimates for services, explanation of benefits/claims, and your accumulation amounts for deductibles and out-of-pocket costs. If you are not currently covered, you will be able to register in the Cigna portal as of your effective date of coverage. Visit the website using the button below to register.
You can go online to see the most up-to-date list of medications your plan covers. Go to myCigna App or myCigna.com, click on the Find Care & Costs tab, select Price a Medication, and type in your medication name.
Use Cigna.com/PDL to access your Cigna Advantage 4-tier prescription drug list (all specialty medications covered on tier 4). You can also determine whether there may be lower cost alternatives for your prescriptions.
Helpful Rx Cost Saving Tools & Tips!
Mail Order - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you. You can get more information on how to start receiving mail order drugs by clicking the mail order link below.
Ask your Doctor or Pharmacist – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.
Offering flexible access to thousands of providers – plus programs and services to support your whole health needs – the Open Access Plus (OAP) plan is designed to make it easier for you to get the quality care you need and the savings you want.

Accounts
You have the opportunity to contribute to a variety of savings and/or spending accounts on a pre-tax basis which lowers your taxable income. View the information below for important details and guidelines for the various accounts offered by Yield Solutions Group.
The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible medical, dental, and vision expenses. The HDHP Medical Plan is qualified for an HSA. By contributing to an HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
HSA IRS Maximums are as follows and are on a calendar year basis. The maximums indicated below are a combined maximum for participant and any employer provided funds to the HSA. If you elect the HSA plan mid-year, maximums are prorated based on your effective date in the HSA plan.
$4,150 for a single contract | $8,300 for a two person or family
$1,000 "Catch Up" contribution for those age 55+

The Flexible Spending Account (FSA) allows you to set aside pre-tax dollars to pay for eligible medical and/or dental and vision expenses. The nice part about an FSA is you have access to the full annualized amount on day one!
The election you make cannot be changed throughout the year unless the change aligns with the qualifying event. For example, if you get married, you can increase your contribution, however, you cannot decrease or end your FSA for this type of life event.
There are two types of FSA options:
- General Purpose FSA: To pay for qualifying medical, dental, and vision expenses. This type of FSA is only available if you do not have an HSA.
- Limited Purpose FSA: To pay for qualifying dental, vision, and post-deductible medical expenses. This type of FSA is available for those who have an HSA.
Plan Details are as follows:
- You can contribute up to $3,050 annually.
- At the end of the 2024 plan year, if you have not used all of your FSA funds, a maximum of $640 will carry over into next year's FSA plan (should you elect an FSA for the 2025 plan year).
- You have 90 days after the end of the plan year to submit for expenses that occurred on or before 12/31/2023.
- If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only which can be used for qualifying dental, vision, and post-deductible medical expenses.
- You have access to your full annualized election amount on day one.

The Dependent Care Reimbursement Account (DCRA) allows you to set aside pre-tax dollars to pay for eligible dependent care expenses. Qualifying expenses include daycare, nursery & preschool tuition, summer day camps, and more!
The election you make cannot be changed throughout the year unless the change aligns with the qualifying event. For example, you can increase your contribution if you add another member to your family through birth or adoption, however, you cannot decrease your contribution for this type of life event.
Plan Details are as follows:
- You can contribute up to $5,000 annually or limited to $2,500 if you are married and file separate tax returns.
- The plan includes a Grace Period which allows you to incur and submit expenses using your 2024 funds until March 15, 2025.
- You have 90 days after the end of the plan year to submit for expenses that occurred on or before 12/31.
- You only have access to the funds of which you have paid into the account.
- Any unused funds are forfeited.
Dental
Yield Solution Group's dental plans provide comprehensive coverage to help you and your family maintain good dental health. The dental plans include both in and out of network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.
If you are already enrolled in one of the Cigna plans today and have not already done so, please be sure to register through the Cigna member portal where you can access important information pertaining to your dental plan with Cigna including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the Cigna portal as of your effective date of coverage.
You have the choice of two dental plans - The Core Plan includes a deductible (for basic and major services only), no ortho coverage, higher cost sharing for your basic and major services, and a lower annual maximum - OR - The Buy-Up Plan. The Buy-Up plan has richer OON benefits, orthodontia coverage is included for dependents (through age 19), and has a higher annual maximum
Cigna offers a large dental network: Cigna Total DPPO (Buy-Up) and Cigna Advantage DPPO (Base). Plus, you will not be balance billed if you see a PPO or Premier dentist - giving them added savings. If you do choose to visit a nonparticipating dentist you may be balance billed and may have to pay more.

Vision
New for 2024! Yield Solutions Group offers vision coverage through Ameritas to help pay for eye exams, prescription glasses and contact lenses. Ameritas also provides discounts for Hearing Care and Lasik.
If you enroll in dental, be sure to register through the Ameritas member portal where you can access important information pertaining to your vision plan with Ameritas including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the Ameritas portal as of your effective date of coverage.
Ameritas allows you to choose between the EyeMed or VSP network. See the below FAQ sheet to select the network that works best for you. You may use the eye care professional of your choice. However, when you visit a participating in-network provider, you receive higher levels of coverage. If you choose to receive services out-of-network, you may be required to pay that provider at time of service and will need to submit a claim to Ameritas for the applicable reimbursement based on the out-of-network reimbursement schedule.

Financial Protection
Yield Solutions Group understands the importance to you and your family to have strong financial protection. We provide all employees Basic Life/AD&D and Short Term Disability coverage at no cost to you. In addition to these benefits, we offer a variety of insurance you can choose from on a voluntary, employee paid basis so you can select what is best for your family. All of these coverages are provided to protect your income and out of pocket expenses.
A sudden accident or death can leave you or your loved ones in a vulnerable position. Yield Solutions Group provides all eligible employees Basic Life and Accidental Death & Dismemberment (AD&D) coverage at no cost to you. Employees have the opportunity to enroll in additional Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
- Evidence of Insurability (EOI) is required for increases in excess of guaranteed issue during open enrollment and for those electing voluntary life for the first time after initially waiving the coverage.
- Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
- *Guarantee Issue only applies to new hires or newly eligible spouses (due to marriage) electing benefits for the first time; or, when increasing benefits during open enrollment. You are eligible to increase your benefits by 1 increment, not to exceed the guarantee issue amount, during the 2023 open enrollment period with no evidence of insurability.

Everyday illnesses or injuries can interfere with your ability to work, more serious issues can impact your ability to work indefinitely. Even a few weeks away from work can make it difficult to manage household costs. Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.
Yield Solutions Group provides a competitive benefits package for all employees as part of our total compensation package. We realize you make benefit choices based on what’s important to you and the needs of your changing lifestyle. Therefore, along with our core benefits, we offer these additional benefits which help to provide financial support to you. These benefits are paid for through payroll deduction on a post-tax basis so that the benefits are tax free.
Accident, Critical Illness, & Hospital coverage is provided through Cigna. Hopefully you are never in the position to use it, but if you were to experience an accident, illness, or hospitalization, it could end up costing you a lot of money. With these plans, you will be paid money for qualifying situations/services to cover our of pocket expenses or you may use the funds for whatever you like.
