Please review our 2023 benefit materials below for information on benefits available to you. Helping you and your family reach and maintain good health is very important to Workforce Software. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

Enrollment

New Hires: You have 14 days from Date of Hire to make your benefit elections.


Remember: The benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.    


You can, however, modify your elections under certain circumstances, called Qualifying Life Events. These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child. Refer to your Employee Benefits Guide or contact Human Resources if you have questions about qualifying events.

BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. A Health Savings Account (HSA) is a tax-free savings account is owned by you, is 100% vested from day one, and lets you build up savings for future needs. The funds may be used to pay for qualifying healthcare expenses not covered by insurance or any other plan for yourself, your spouse, or tax dependents. You decide how much you would like to contribute, when and how to spend the money on eligible expenses, and how to invest the balance.



Contribution limits include your individual contribution as well as any employer contributions. The combined total cannot exceed the amount(s) mentioned above each calendar year. If you need to change your election amount, see Carol Duman.

The Health Care, Limited Purpose and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to an FSA you reduce your taxable income, so you pay less in taxes — which saves you money.


HEALTH CARE FSA

  • You may contribute up to $3,050 annually
  • All benefit eligible can participate, unless you or your spouse are contributing to an HSA
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses
  • You can carry over up to $610 of unused funds into the next plan year (this amount does not count towards the IRS maximum set for the plan year)
  • You have 90 days after the end of the plan year to submit for reimbursement for expenses incurred in the previous plan year. For example, in February 2023 you can submit a request for reimbursement for an eligible expense that was incurred in December 2022 - the expense cannot have been incurred in the current plan year


LIMITED PURPOSE FSA

  • You may contribute up to $3,050 annually
  • If you are enrolled in an HDHP HSA– you are only able to participate in a Limited Purpose FSA
  • Limited Purpose FSA can over cover the costs for qualified dental and vision expenses for yourself and dependents – not able to cover any qualified medical expenses
  • You can carry over up to $610 of unused funds into the next plan year (this amount does not count towards the IRS maximum set for the plan year)
  • You have 90 days after the end of the plan year to submit for reimbursement for expenses incurred in the previous plan year. DEPENDENT CARE FSA

  • You may contribute up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns
  • Eligible expenses include care for your dependent child, spouse, parent or grandparent who are incapable of caring for themselves
  • You have a 2.5 month grace period after the end of the plan year to use any remaining funds - there is no carryover option for these accounts

We provide all team members with Basic & Voluntary Life/AD&D coverage through Prudential.


Basic Life/AD&D

Employees working 30 or more hours per week are eligible for 1x your base annual earnings, to a maximum of $50,000, in coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.


Voluntary Life/AD&D*

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


*Evidence of Insurability (EOI) Reminder

Workforce Software provides you with the opportunity to buy up Life Insurance. As an employee you are able to buy up to a total of $500K, and for a spouse you can buy up to a total of $500K or 50% of what you elect on yourself. Anything over $200K as the employee or $50K as the spouse you will need to provide Evidence of Insurability (EOI) to Prudential. To provide this information please follow the link below and complete the information requested. Prudential will receive the information and will approve/disapprove.  The cost will not be charged to you until it has been approved. it is your responsibility to complete the EOI within 30 days of your election or it will become null and void. 



Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

We provide all team members with Short- and Long-Term Disability through Prudential.


Short-Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short-Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Your coverage through Prudential pays 60% of your income to a maximum of $1,500 per week.


Long-Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long-Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits begin after a 180-day elimination period are equal to 60% of covered monthly earnings up to $10,000 for full-time employees.
