Wolverine Human Services takes into consideration our employees' evolving needs, as well as ensuring a level of security and protection when making decisions regarding the benefits program being offered. We recognize the important role employee benefits play as a critical component of an employee's overall compensation. We also strive to maintain a benefits program that is competitive within our industry.
For 2023, Wolverine Human Services received a 33% increase in overall healthcare cost. In order to control these costs, we had to make a few changes to benefit designs and contributions. It is still important for Wolverine Human Services to offer a competitive benefit package to our valued team members and in doing so Wolverine Human Services absorbed 23% of the increase.

Before you enroll, please read this guide to become familiar with the benefit options. Your decisions will impact your benefit selections and what you pay for these benefits.
You are eligible to participate if you are full-time and work a minimum of 30 hours per week. Your coverage will be effective on the 91st day of full-time employment. Part-Time, seasonal, temporary, internship, and contracted employees are not eligible to participate.
Children are eligible for benefits until the last day of the calendar year in which they turn 26.
You will be required to complete Enrollment during the New Hire Orientation.
ENROLLMENT INSTRUCTIONS: Employee Navigator (New Enrollment Tool)
The Benefit choices you make during your initial enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Life Events." These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a Qualifying Life Event, you may make changes to your benefits by contacting Human Resources and completing any election changes in Employee Navigator within 30 days of the event. You will need to provide supporting documentation to support the changes based on the Qualifying Life Event, these should be submitted to Megan or Katrina.

For those employees who are covered by one of the BCN medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provides members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.
How Much Does It Cost?
HMO 1500: Equal to your office visit copay.
HSA Plan(s): Services will be subject to your deductible and coinsurance; applicable charges until you have satisfied your deductible are as follows: Virtual Medical Visit $59; Virtual Therapy $85-$99; Virtual Psychiatry $199 for initial visit and $95 for follow up visits.
Prescriptions: If prescribed during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in.

Our Ameritas dental plan provides comprehensive coverage to help you and your family maintain good dental health. Please note that if you go to an out-of-network provider, your costs will be higher.

We offer vision coverage through NVA (National Vision Administrators) to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations.

Pre-Tax Contributions
You share in the cost of your Medical, Dental and Vision Benefits. Your employee contributions for these benefits are taken out of your pay on a pre-tax basis as set forth below. In addition, if you elect to contribute to a flexible spending account, those contributions will be taken out of your pay on a pre-tax basis based on your election. Contributions to flexible spending accounts and health savings accounts are also taken on a pre-tax basis.
After-Tax Contributions
If you choose to purchase the following additional benefits, your contribution will be taken out of your pay on an after-tax basis:

The HealthEquity Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:

$7,750 for a two person or family contract
$1,000 "Catch Up" contribution for those age 55+

The iSolved Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.

We are proud to provide you with financial protection benefits through Lincoln Financial Group with Basic Life insurance, Accidental Death & Dismemberment (AD&D) insurance. Wolverine Human Services covers a portion of your Short-Term Disability coverage. You also have the opportunity to purchase Supplemental Life insurance.
Basic Life/AD&D
You are automatically enrolled in Basic Life and AD&D insurance benefits. Your Basic Life insurance benefit is $10,000 and the amount of your AD&D benefit is $10,000. You do not need to enroll in these benefits as they are provided at no cost to you.
Supplemental Life
You have the opportunity to enroll in Supplemental Life insurance for yourself, your spouse or your dependent children, which will supplement lost income in the event of death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage. You must elect Supplemental Life insurance coverage in order to elect coverage for your spouse and/or children.
Short Term Disability
To ensure your income will continue if you are unable to work due to a disability that extends for more than 14 consecutive days, Wolverine Human Services offers short-term disability (STD) for purchase through Lincoln National. Benefits are payable for a non-occupational injury or illness that keep you from performing the normal duties of your job. If a medical condition is job-related, it is considered Workers’ Compensation rather than STD. If you elect this coverage, you will be responsible for 70% of the cost, Wolverine will cover the remaining 30%.
The following Voluntary Benefits offered by LFG can complement existing medical coverage and help fill financial gaps caused by out-of-pocket expenses such as deductibles, co-payments, and non-covered medical services. Benefits are paid regardless of what is covered by medical insurance. Payments are made directly to you, to spend as you choose. All plans are portable, which means you can continue coverage if you leave the company.
Accident Insurance
Accident Insurance is designed to help covered individuals meet the out-of-pocket expenses and extra bills that can follow an accidental injury, whether minor or catastrophic. Lump sum benefits are paid directly to you based on the amount of coverage listed in the schedule of benefits. The coverage is guaranteed issue, so no health questions are required.
Critical Illness
Critical Illness Insurance is designed to help you offset the financial effects of a catastrophic illness with a lump sum benefit if you are diagnosed with a covered critical illness. The benefit is based on the amount of coverage in effect on the date of diagnosis or the date treatment is received according to the terms and provisions of the policy. You have the choice of electing coverage of $10,000, $15,000 or $20,000. Your spouse and child(ren) will each be offered 50% of your benefit amount.
Hospital Indemnity
Hospital Indemnity insurance is designed to help provide financial protection by paying a benefit due to a hospitalization. You may use the benefit to meet the out-of-pocket expenses and extra bills that occur.
