EMPLOYEE BENEFITS
Helping you and your family reach and maintain good health is very important to Wireless Vision. Wireless Vision is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Wireless Vision remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees. Below are highlights of the new changes that will be effective January 1, 2023:
Medical plans have been consolidated for simplicity and efficiency - we now offer the POS $1,500, POS $2,500, and POS HSA $2,000
Small increases in contributions for Medical, Dental and Vision.
Please review your 2023 Wireless Vision Open Enrollment materials for more information regarding your plans offerings for 2023.
ELIGIBILITY
EMPLOYEE ELIGIBILITY
Salaried and Corporate employees are eligible First of the Month following Date of Hire. Hourly, non-exempt, employees are eligible on the 1st of the month following 60 days of employment.
If you work a minimum of 20 hours per week you can:
- Select Health, Dental, Vision and Voluntary Group Accident Insurance
- Participate in our Flexible Spending Accounts
If you work a minimum of 30 hours per week or more you can:
- Select Health, Dental, Vision, Voluntary Group Accident Insurance and Voluntary Group Critical Illness
- Participate in our Flexible Spending Accounts
- Receive company paid Group Life and AD&D Insurance and Short-Term Disability (STD) and Long-Term Disability (LTD) coverage, including the LTD Buy-Up option.
DEPENDENT ELIGIBILITY
You may also enroll eligible dependents for benefits coverage. A ‘dependent’ is defined as the legal spouse or registered domestic partner and/or ‘dependent child(ren)’ of the plan participant or the spouse.
- Dependents are eligible up until the end of the month they turn 26
QUALIFYING LIFE EVENTS
- You have 30 days to report the change in Ultipro when you have a Qualifying Life Event
- Common life events include; Marriage, Divorce, New Dependent, Loss/gain of available coverage by you or any of your dependents.
- You may only make changes directly related to the event – you may not change benefit options

ENROLLMENT INSTRUCTIONS:
- Go to https://wvgo.wirelessvision.com
- Under the Quicklinks go to Ultipro
- Under the main menu select Manage My Benefits
- Start your Enrollment
- Verify your personal information. If your information is incorrect go back to the main menu and update your personal information under the Personal section.
- Select your dependents (if applicable)
- Make your elections. You must go through and confirm your elections
- Designate your beneficiaries
- Submit your elections. Once you reach the end you will have the ability to print a confirmation page, please print a copy for your records.
- Upload copies of any requested documents to the system. Your elections will not be approved if documentation is not received within 14 days.

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
- Covered by a Qualified High Deductible Health Plan
- You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
- You cannot be covered by Medicare or Medicaid
- You cannot be eligible to be claimed as a tax dependent
- You have not received VA benefits in the past 90 days
2023 HSA IRS Maximums are as follows:
$3,850 for a single contract
$7,750 for a two person or family
Additional $1,000 "Catch Up" contribution for those age 55+
2023 Wireless Vision Funding:

Double or Family: $520/year

Wireless Vision provides those employees who are covered by one of the Aetna medical plans access to virtual visits with Teladoc. Teladoc provides members with the opportunity to receive medical and mental health care from the comfort of your own home. With Teladoc, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.
How Much Does it Cost?
- Standard Plan (Non HDHP/HSA): Equal to your office visit copay.
- HDHP/HSA Plan: Services will be subject to your deductible and coinsurance; applicable charges until you have satisfied your deductible are as follows: Virtual Medical Visit $59; Virtual Therapy $85-$99; Virtual Psychiatry $199 for initial visit and $95 for follow up visits.
- Prescriptions: If prescribed during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in.

MINUTECLINIC
AETNA MEMBERS CAN ACCESS CONVENIENT, HIGH QUALITY, AFFORDABLE CARE WITH NO COPAY UNDER THE PPO 500, 1500 & 2500 PLANS. A MINUTECLINIC VISIT WILL BE SUBJECT TO DEDUCTIBLE & COINSURANCE UNDER THE PPO HSA 2000 PLAN.
- MinuteClinic is a walk-in clinic inside select CVS Pharmacy and Target stores
- MinuteClinic health care providers treat a variety of illnesses, injuries and conditions, and can also write prescriptions when medically appropriate
- Provides a low-cost alternative when a member’s Primary Care Provider is unavailable or they do not have one
- Lower cost than ER and urgent care centers
- Both walk-in and scheduled appointments available
- Affordable weekend and after-hours care

Wireless Vision offers you the choice of two Dental plans offered through Delta Dental of Michigan. Consider how you would use your dental coverage, preferred dentist and then make your selection.
•The Value Plan does not have a deductible, but you must use a PPO dentist to have coverage
•The Plus Plan includes a deductible, offers in-and-out of network coverage and has an out-of-pocket maximum
•Members enrolled in Value Plan have access to the PPO Network, while Plus Plan members have access to the PPO and Premier networks

Vision Insurance is offered through EyeMed. There are two plan options – High and Low – to find the coverage that best meets your need. Under these plans, you may use the eye care professional of your choice. However, when you visit a participating in-network provider, you receive higher levels of coverage. If you choose to receive services from an out-of-network provider, you will be required to pay that provider at the time of service and submit a claim form for reimbursement.

Wireless Vision provides you with financial protection options for life, accidental death & dismemberment, short-term disability and long-term disability with Unum.
Voluntary and Employer Paid Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Employer Paid Short-Term Disability
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short-Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long-Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.
