ELIGIBILITY AND ENROLLMENT
Eligibility
All full-time employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Whitesell. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.
Enrollment
Action Required: Please submit your elections within 14 days of your hire date.
ENROLLMENT INSTRUCTIONS:
- Go to www.WhitesellConnection.com
- Enter your User ID: First Initial + Last Name (ex.: John Smith = JSmith)
- Enter your Password: Last 4 of your SSN
- Follow instructions and enroll in your benefits
- Make sure to save your elections and print your confirmation statement.
The Benefit choices you make will remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

We continue to offer three medical and prescription drug options in 2023, all through Blue Cross Blue Shield of Michigan (BCBSM):
- PPO 1000
- PPO 3000
- PPO HSA 3500
All of the options are PPOs. A PPO option offers the freedom to see any provider when you need care. When you use providers from within the PPO network, you receive benefits at the discounted network cost. Most expenses, such as office visits, emergency room and prescription drugs are covered by a copay. Other expenses are subject to a deductible and coinsurance.

VIRTUAL VISITS

Whitesell Corporation provides those employees who are covered by one of the medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.

How Much Does it Cost?
- PPO 1000: $40 copay per visit

- Prescriptions: If prescribed during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in.

HEALTH SAVINGS ACCOUNTS

If you enroll in the PPO HSA 3500 option, the Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money. In addition, Whitesell will contribute to your HSA.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
- You must be covered by a Qualified High Deductible Health Plan
- You cannot be covered by another health plan (including if your spouse has a health care Flexible Spending Account)
- You cannot be covered by Medicare or Medicaid
- You cannot be eligible to be claimed as a tax dependent
- You must have not received VA benefits in the past 90 days
HSA IRS Maximums for 2023 are as follows (any employer contributions to the HSA will apply to the maximum noted below):
- $3,850 for a single contract
- $7,750 for a two person or family contract
- $1,000 "Catch Up" contribution for those age 55+
Whitesell will contribute to your HSA as follows:
- Single: $400 annually
- Two Person: $800 annually
- Family: $1,000 annually
DENTAL

Dental Benefits through Delta Dental of Michigan provide comprehensive coverage to help you and your family maintain good dental health. Your coverage will be greater if you go to an in-network provider. You have two options: the Delta High Plan and the Delta Low Plan. A summary of each option is below.

Whitesell Corporation offers Vision coverage through Eyemed to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. You have two options: the Eyemed High Plan and the Eyemed Low Plan. A summary of each option is below.

Whitesell Corporation provides you with financial protection benefits with Basic Life insurance, Accidental Death & Dismemberment (AD&D) insurance, Voluntary Short Term Disability and Voluntary Long Term Disability benefits with Mutual of Omaha. You also have the opportunity to purchase Supplemental Life insurance.
Life Insurance, AD&D and Supplemental Life
Basic Life/AD&D
You are automatically enrolled in Basic Life and AD&D insurance benefits. Your Basic Life Insurance benefit is 2x your Basic Annual Earning, $500,000 maximum (reduced if you are age 65 or older), and the amount of your AD&D benefit is equal to your Life insurance. You do not need to enroll in these benefits as they are provided at no cost to you.
Supplemental Life
You have the opportunity to enroll in Supplemental Life insurance for yourself, your spouse or your dependent children, which will supplement lost income in the event of death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage. You must elect Supplemental Life insurance coverage in order to elect coverage for your spouse and/or children.
- Employee: Choose in $10,000 increments up to the lesser of 5x your annual salary or $500,000. Guarantee Issue: $180,000
- Spouse: Choose in $5,000 increments up to 100% of the amount you elect for yourself to a max of $250,000. Guarantee Issue: $25,000
- Child: Choose in $2,000 increments up to $10,000. Guarantee Issue: $10,000
Evidence of Insurability (EOI): When you are first eligible for Optional Life and AD&D, you may purchase up to $180,000 Guaranteed Issue (GI) for yourself and up to $25,000 for your spouse without providing proof of good health (EOI). Any amount elected over the GI will require EOI. If you elect optional life coverage, and are required to complete an EOI, it is your responsibility to complete the EOI and submit it to Human Resources within 30 days of your election. Employees who request coverage at a later date are subject to EOI. In addition, your spouse will need to provide EOI to be eligible for coverage amounts over GI, or if coverage is requested at a later date.
Who's Your Beneficiay? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Voluntary Disability Benefits
Short Term Disability
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. You have the ability to purchase Short Term Disability coverage that will provide you 60% of your base weekly (up to $1,000 per week) for up to 12 weeks.
Typical claims would include: pregnancy, injuries, joint, back and digestive disorders.
Long Term Disability
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. You have the ability to purchase Long Term Disability coverage that will provide you with 60% of your monthly wages up to a maximum benefit of $5,000 per month.
Typical claims would include: cancer, back disorders, injuries and poison, cardiovascular, joint disorders.
