OPEN ENROLLMENT
Open enrollment is your annual opportunity to update and learn about the benefits provided through Warwick Investment Group. We are happy to announce exciting changes that will reduce premium cost and improve our benefit offerings.
Benefit highlights and changes for the 2026 plan year:
- New Carriers - United Healthcare (UHC) will replace BCBS for medical and Mutual of Omaha will replace Principal
- Enhanced High Dental Plan - Will now include adult orthodontia coverage and an increased annual benefit maximum
- Supplemental Life and AD&D - Lower premium cost and you can elect up to the guarantee issue this open enrollment without submitting medical questions.
- Long-Term Disability - Increased monthly benefit payout maximum
- New Voluntary Benefits - Accident, Critical Illness and Hospital Indemnity
- HSA Employer Contribution: Warwick will make contributions each pay period rather than quarterly and has increased the Employee + Dependent contribution
- Everyone is required to review and confirm benefits through Paycom between November 17th and December 3rd
Benefit changes outside of Open Enrollment require a
Qualifying Life Event and/or Loss of Eligibility.
You may not change your benefits throughout the year unless you experience a Qualifying Life Event. Qualifying Events may include marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, that requires a benefit change, you must notify report through Paycom within 30 days of the event. Please remember you only have 30 days from the event to make changes to your benefit elections.
MEDICAL & Rx
Moving Carrier Tips:
- Check if regularly seen providers are in the UHC Choice Plus network.
- The Choice Plus network is national and has a 99.69% match to our providers.
- Fill maintenance medications before January 1st, if possible, in case of implementation delays.
- Digital ID information can be found at www.myuhc.com once implementation is complete.
- New ID cards will be mailed to employee's home addresses.
HSA
Health Savings Account
- Only available when enrolled in a High-Deductible Health Plan (HDHP).
- Ineligible if:
- you are enrolled in Medicare, Indian Health Services or Military (Tricare) Coverage.
- your spouse is enrolled in a Flexible Spending Account (FSA).
- you can be claimed as a dependent on someone's tax return.
- A true bank account in your name.
- Contributions are pre-tax dollars automatically put in your account (employer and employee contributions). If funds cannot be contributed to your account by your employer, then your employer contribution may be forfeited, and your payroll deductions/contributions will be returned to you through payroll.
- Funds are available as they are contributed per pay period.
- Funds are used to pay for qualified medical, dental, vision and some over the counter expenses tax free.
- Unused funds rollover from year to year and accumulate tax free and can be invested through Health Equity.
- At age 65 you can use your funds for non-qualified expenses but will pay income tax.
FSA
Healthcare - Flexible Spending Account
- 2026 Annual contribution limit: $3,400
- Paired with traditional medical plan (non-HDHP).
- Full elected amount is available the first day of the plan year.
- Funds are used to pay for out-of-pocket medical, dental, vision and some over the counter items tax free.
- At the end of 2025 funds over $660 will be forfeited. This will go up to $680 in 2026.
- Funds $660 and under will rollover to next plan year after 90 days.
Dependent Care - Flexible Spending Account
- 2026 Annual contribution limit: $7,500 or $3,750 if you are married and file taxes separately from your spouse.
- Anyone is eligible with dependents who need care while guardians are working.
- Funds pay for dependent care (children under 13 years or disabled dependents) tax free.
- Funds are available as they are contributed per pay period.
- Unused funds will be forfeited (runout period to file claims: 90 days after plan year ends).
DENTAL
Dental Tips:
- Out-of-network benefits pay the same as in-network.
- Using in-network providers will give you the lowest out-of-pocket cost.
- Dental ID cards will be sent to home addresses and can be accessed online at mutualofomaha.com/dental.
VISION
Utilizes the EyeMed Network
LIFE AND AD&D
Basic Life and AD&D
Warwick provides all full-time employees with 1x annual salary up to $200,000 in Life and AD&D.
Reminder: Update your beneficiaries at any time during the year through Paycom.
Voluntary Life and AD&D
Per pay period cost will be calculated on employee's age and election amount(s). In order to elect supplemental life for your spouse and/or child(ren), employees must elect coverage for themselves.
DISABILITY
Disability insurance helps protect your income if you are not able to work due to injury or illness. These benefits are provided by Warwick at no cost to full-time employees.
Short-Term Disability
This benefit will pay you 60% of your weekly income up to $2,000 per week after 7 days from a disabling illness and 0 days from disabling injury. STD will last until you can return to work or up to 13 weeks.
Long-Term Disability
This benefit will pay you 60% of your monthly income up to $7,500 per month after 90 days from a disabling illness or injury. LTD will last until you can return to work, reach 65 or social security retirement age.
NEW VOLUNTARY BENEFITS
All three of these polices provide a cash benefit paid directly to you to help during a time when out-of-pocket medical and life costs accrue.
Accident
This insurance pays a benefit for each injury, treatment or service included in the policy that occurs as a result of a covered accident. See full benefit summary below for complete policy coverages.
Plan Examples:
- Emergency Room: $300
- Urgent Care Center: $225
- Fractures (Surgical / Non-surgical): Up to $12,000 / Up to $6,000
- Health Screening Benefit: $50 per year up to 6 per family
Critical Illness
A critical illness policy provides a lump-sum cash benefit upon diagnosis of a critical illness like a heart attack, stroke or cancer. The benefit can be used to pay out-of-pocket expenses or to supplement your daily cost of living.
- Employee Benefit: $5,000 up to $50,000 - $20,000 Guarantee Issue
- Spouse Benefit: $1,000 up to $25,000 not to exceed 100% of employee election - $10,000 Guarantee Issue
- Free Child(ren) Benefit: 25% of employee election up to $13,000
- Health Screening Benefit: $50 per year up to 6 per family
- Premium is based on benefit amount and employee age.
Hospital Indemnity
A hospital indemnity insurance policy supplements your medical coverage and provides a cash benefit for hospital related fees you or an insured family member sustain as a result of being hospitalized.
- Hospital Admission: $1,000
- Daily Hospital confinement: $100 per day
- Daily Newborn Nursery Care Confinement: $75 per day (up to 2 days per year)
- Health Screening Benefit: $50 per year up to 6 per family
