Helping you and your family reach and maintain good health is very important to WorkQuest.
WorkQuest is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind.
We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.
You can, however, modify your elections under certain circumstances, called "Qualifying Events".
These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan.
If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.
EMPLOYEE ELIGIBILITY
You are eligible to participate if you are full-time and work a minimum of 30 hours per week.
Your coverage will be effective the 1st of the month following 30 days from your date of hire.
Now is a great time to update your Beneficiary Designation for Dearborn Life Insurance and Voya Retirement Plans.
Access both Voya and BCBS/Dearborn beneficiary designation forms below if you would like to make any beneficiary changes.

MEDICAL | WELLNESS
Medical Coverage is provided to ALL eligible Employees at NO COST
You may choose to cover your qualified dependents through Payroll deductions.
MEDICAL COVERAGE Cost beginning August 1, 2023
Employee: $0
Employee + Children: $149.34 per pay period
Employee + Spouse: $242.66 per period
Employee + Family: $389.26 per pay period
PPO | In-Network & Out-of-Network Benefits Available
The PPO option offers the freedom to see any provider when you need care.
•When you use providers from within the PPO network, you receive benefits at the discounted network cost.
•Most expenses, such as office visits, emergency room and prescription drugs are covered by a copay. Other expenses are subject to a deductible and coinsurance.

BCBS WELLNESS
Wellness Programs and Incentives brought to you by BCBS of Texas.
You can access these benefits by registering on the BCBS website or downloading the BCBS mobile app.
DENTAL - 100% Employer Paid
Dental Coverage is provided to ALL eligible Employees and qualified Dependents at NO COST
The WorkQuest dental plan is with BlueCross BlueShield of Texas provides comprehensive coverage to help you and your family maintain good dental health.
You may select the dentist of your choice; however, when you visit a participating in-network dentist you could have lower out-of-pocket costs,
no balance billing, and claims will be submitted by your dentist on your behalf.

VISION - 100% Employer Paid
Vision through BCBS - EyeMed Vision care network
Vision coverage is provided to ALL eligible employees and qualified Dependents at NO COST
This plan assists with copayments for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in the EyeMed Network.

- Babies should receive their first professional eye exam at 6 months

LIFE Insurance | Voluntary Life Insurance
WorkQuest provides all eligibile Employees with valuable Life Insurance coverage at NO COST
You may also purchase additional Supplemental Life coverage through Payroll deductions for yourself and your Dependents.
Group Term Life/AD&D Benefit:
This plan benefit includes life coverage amounts of 3 times your Annual Earnings to a maximum of $500,000, plus an additional $100,000.
Life insurance is an important part of your financial security. Life insurance helps protect your family from financial risk and sudden loss of income in the event of your death. AD&D insurance is equal to your Life benefit in the event of your death being a result of an accident and may also pay benefits for certain injuries sustained.
Who is your Beneficiary?
Make sure to designate a beneficiary and to update the Beneficiary should you experience any life events that may require a change to your Beneficiary Designation.

DISABILITY | EAP
WorkQuest provides Long Term Disability benefits to ALL eligible employees at NO COST.
Disability benefits are designed to replace a portion of your income if you are unable to work due to an accident or illness.
Please review the LTD plan summaries below for specific coverage information.
An additional feature of this Disability coverage includes access to the
Employee Assistance Plan through BCBS/Dearborn’s Disability Resource Services.

COLONIAL
Life is full of challenges... Protecting your way of life shouldn’t be one.
If something happened to you, would your family have the income they need?
WorkQuest provides a Medical Bridge policy through Colonial Life to all eligible Employees at NO COST.
You may also purchase this benefit for your Dependents through payroll deductions.
Medical Bridge coverage provides first dollar benefits for various Medical expenses.
- Hospital Confinement - $500 once per year for a minimum 20 hour stay
- Out Patient Surgery - $500 or $1000 per surgery depending on tier for a total of $1500 per year
- Diagnostic testing - $250 once per year
- Emergency Room Visit - $150 once per year
- Wellness Benefit - $50 once per year
Cancer Assist - You may purchase this coverage through payroll deductions for yourself and your dependents
The Cancer Assist coverage helps you pay for some of the direct and indirect costs related to cancer diagnosis and treatment.
- Includes $5,000 initial diagnosis benefit, specified disease rider and annual wellness benefit of $50.
With both the Medical Bridge and Cancer Assist policies benefits are paid directly to you.
