Remember all elections are binding through 12/31/2024 unless you have a valid qualifying life event such as marriage, the birth of a baby, or a divorce. You must submit proof of a qualifying life event within 30 days of the event to HR in order to make a change related to the qualifying life event.

Benefits Overview

WHAT'S CHANGING


Base Plan

· Moving to a Qualified High Deductible Health Plan – All non-preventative services will now be subject to the deductible

· *Enhancement* The deductible will be waived for select maintenance medications for conditions such as Asthma, Depression, Diabetes, Hypertension, High Cholesterol, and more


Deductible: $3,000 / $6,000

· You will pay copays for services after your deductible is met

· Employee enrolling in this plan are eligible to open Health Savings Accounts (HSAs), which are similar to Flexible Spending Accounts, but with some extra advantages such as:

o  Higher contribution limits: $4,150 (single) / $8,300 (family)

o  Change your election any time

o  Balances roll over each year and are portable


Mid Plan

·        There will no longer be a mid-plan offered


High Plan

· The existing High Plan will no longer be offered

· Deductible: $2,000 / $4,000

· The deductible will be waived for office visit copays and select maintenance medications for conditions such as Asthma, Depression, Diabetes, Hypertension, High Cholesterol, and more

· You will pay copays for services after your deductible is met


Vision

You can now pick between the VSP and Eyemed network. Slight differences on each plan.


WHAT'S NOT CHANGING

Dental: Delta Dental:


If your dentist is in the Delta Dental Premier network, the % of the benefit covered will be at the out of network level.

Unlike Out of Network dentists, you cannot be balance billed by Delta Dental Premier dentists.

Balance billing is when a provider does not accept the insurance carrier payment as payment in full and bills you the

difference. For example, if you have a $150 preventive charge and Delta's negotiated rate is $100, you will pay $0 with a

Delta Dental PPO and Delta Dental Premier dentist. However, with an out of network dentist, you could be charged the

remaining $50.

Choose the level of protection that best fit your and your family’s needs. All plans include 24/7 customer support, $1M per adult identity theft insurance, monitoring of USPS address, personal information, financial and online accounts, SSN authentication alerts, antivirus protection for 1 device, and 1 bureau credit monitoring. Protection Plus and Executive plans are more comprehensive such as antivirus protection on more devices and credit monitoring from more bureaus. How To Enroll

Action Required

Beginning December 5th, Employees must go to the UKG/Kronos Workforce Ready Employee Portal to enroll in benefits or make changes if you have a qualifying life event. The UKG/Kronos portal will be open from December 5th through Midnight December 15th.


Your decisions will remain in effect for the entire plan year (January 1, 2024 to December 31, 2024) unless you experience a qualifying life event.
