Medical

Both medical options are PPOs. A PPO option offers the freedom to see any provider when you need care. When you use providers from within the PPO network, you receive benefits at the discounted network cost. Most expenses, such as office visits, emergency room and prescription drugs are covered by a copay. Other expenses are subject to a deductible and coinsurance.

A comparison of the options is below:

Prescriptions

The differences in the prescription drug costs are summarized here:

In addition, if you need a specialty medication, you can contact AllianceRx Walgreens Prime, which will support you with:

  • scheduling delivery times and locations
  • finding financial assistance options
  • assisting with prior authorizations
  • managing medication side effects

Health Care Flexible Spending Account

Health Care FSA

The maximum amount you can contribute to the Health Care FSA is $3,050. If you do not use all of your Health Care FSA dollars during the calendar year, you can roll over up to $550 to the next calendar year. In general, all eligible health care expenses can be reimbursed. However, if you are enrolled in the PPO HSA medical option, you will be enrolled in a Limited Purpose FSA, which only reimburses for eligible dental and vision expenses.

Dependent Care Flexible Spending Account

You can contribute up to $5,000 annually, $2,500 if you are married and file separate tax returns. Reimbursements are allowed for care for certain dependent children and for a spouse, parent or grandparent that is incapable for caring for themselves. Any unused funds do not rollover to the next year.

Life and AD&D

Basic Life/AD&D

You are automatically enrolled in Basic Life and AD&D insurance benefits. Your Basic Life Insurance benefit is 1 x your annual earnings, with a minimum benefit of $50,000 and a maximum benefit of $300,000 (reduced if you are age 70 or older), and the amount of your AD&D benefit is equal to your Life insurance. You do not need to enroll in these benefits as they are provided at no cost to you.

Supplemental Life

You have the opportunity to enroll in Supplemental Life insurance for yourself, your spouse or your dependent children as well as Optional AD&D insurance for yourself, your spouse or your dependent children. If you choose to enroll in employee coverage, this will be in addition to your employer provided coverage. You must elect coverage for yourself in order to elect coverage for your spouse and/or children.

Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

Disability

Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. You have the ability to purchase Short Term Disability coverage that will provide you 60% of your weekly income (up to $1,500 per week) for up to 26 weeks.

Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Airboss provides you with a Long Term Disability benefit, that covers 66.67% of your monthly income up to a maximum benefit of $12,500 per month.
