WELCOME AND INTRODUCTION

Helping you and your family reach and maintain good health is important to MP Technologies. MP Technologies is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by MP Technologies. If you choose to enroll in the benefits program, you may also cover your eligible dependents. Eligible dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.

To review your benefits, please download and review the 2024 Benefit Guide. This guide will include a summary of the benefits MP offers along with premium costs for 2024. It is important to review this guide along with the summary of benefits included on this site in order to understand each benefit and its offering.

To access your applicable plan documents, please visit the below link and log into your Webapps account. Click on the Human Resources tab, then Benefits.

OPEN ENROLLMENT

Annual Open Enrollment will be November 1st - November 30th, 2024.

Action Required

UKG will be used to complete your 2025 benefit elections. Be sure to make your elections in UKG within 30 days following your eligibility date. If you do not make elections, then you may not be able to get coverage until the next open enrollment period.

The benefit choices you make during your initial enrollment will remain in effect for the entire year.

You can, however, modify your elections under certain circumstances, called "Qualifying Events". These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

LIFE & DISABILITY

You have the option to enroll in Voluntary Life/AD&D coverage through MetLife.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Voluntary Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. You have the option to enroll in Short Term Disability coverage through MetLife. Benefits begin after a 14 day waiting period. The benefit pays 60% of your income to a maximum of $2,500 per week.


Voluntary Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. You have the option to enroll in Long Term Disability coverage through MetLife. Benefits begin after a 90 day elimination period. Benefits are equal to 60% of covered monthly earnings up to the stated maximum based on your employment classification.
