Union Employees have three medical options in 2025:
If you're enrolling for the first time, changing plans, or removing/adding a dependent please complete the IHA Enrollment Application.
You can elect to deposit funds into an FSA if you take the Encompass C or Encompass D plan.
You can elect to deposit funds into the HSA if you take the iDirect $1,650/$3,300 plan.
If you're enrolling for the first time, changing plans, or removing/adding a dependent please complete the Ameritas Enrollment Application.
If you're enrolling for the first time, changing plans, or removing/adding a dependent please complete the Davis Vision Enrollment Application.
If you're interested in electing Voluntary Life, please complete the enrollment and the EOI form.
