Please review your current benefit elections. For your medical or your flexible spending account, verify your personal information and make any changes if necessary.
1. If you are not making changes, your current coverage and network will carry over to the new plan year.
2. If you are making changes, you must complete a change form.
3. If you are enrolling in a benefit you weren't enrolled in last year, you must also complete the necessary carrier enrollment forms.
REMINDER: if you wish to participate in a Flexible Spending Account in 2026, you MUST make an active election each year. Prior elections do not carry over.
Changing Your Elections
The benefit choices you make during your initial or annual enrollment remain in effect for the entire calendar year. You are, however, allowed to modify your elections in certain situations, called “qualifying life events.” If you experience a qualifying life event, as described below, you may make changes to your benefits within 30 days of the event.
Why It Matters
Missing the Special Enrollment Period could mean waiting until the next Open Enrollment to make changes—potentially leaving you or your family without coverage.

Medical Plan Options
Minnesota Masonic Home offers two medical plan options. All options provide high-quality, affordable medical care, including preventive care, doctor's visits, hospitalization, and emergency care. However, each plan has unique characteristics and advantages. You can choose to use providers in-or out-of-network at the time of service. Using in-network providers ensures you will receive the highest level of benefits at the in-network discounted costs.

It is important to choose the network option carefully. You will not be able to make a new network selection until next year’s annual enrollment period. Refer to your Employee Benefits Guide or to the Medica Plan Summaries for additional coverage information.
Network Choices
Medica Choice Passport Network: Medica Choice Passport gives employees access to a national network and the freedom to see any network provider at any time. Members benefit from network discounts and a plan that's easy to use — no matter where they live. Medica Elect Network: The Medica Elect Network is a medium-sized regional network. You choose a primary care clinic and receive care from providers in your care system. The following care systems are included in the Medica Elect Network: Allina Medical Clinics, Minnesota Healthcare Network, Children's Health Network, Park Nicollet Health Services, Hennepin Healthcare, RiverWay/North Suburban Clinics, Integrity Health Network, St. Luke's Care System and Lakeview Medical Care System. Medica Vantage Plus Network: The Medica Vantage Plus Network offers access to more than 4,400 primary and specialty care providers, 655 clinics and 12 hospitals. As one of the largest accountable care organizations in Minnesota, Vantage Plus has the providers you know and trust from M Health Fairview (the health care organization representing Fairview, University of Minnesota and M Physicians), North Memorial Health and many popular independent clinics. Medica Essentia Choice Care Network (Duluth Only): The Medica Essentia Choice Care Network offers access a unique of providers in northern Minnesota, eastern North Dakota and northwestern Wisconsin. This network includes 26 hospitals, over 70 clinics and more than 2,300 physicians and advance practitioners. Flexible Spending Account (FSA)

Can be a great way to save money on taxes and pay for unexpected medical expenses.
Flexible Spending Accounts (FSA) allow you to reduce your taxable income by setting aside pre-tax dollars from each paycheck to pay for eligible out-of-pocket health care and dependent care expenses for yourself, your spouse and your dependent children.
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.
Health Care FSA
Maximum Annual Contribution | $3,400
All benefit eligible can participate, unless you or your spouse are contributing to an HSA.
Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses
Dependent Care FSA
Maximum Annual Contribution | $7,500
Limited to $3,750 if you are married and file separate tax returns
How the Plan Works
You must incur your eligible expenses during the plan year — January 1 to December 31. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service.
Important FSA Rule
Estimate expenses carefully. The federal use-it or lose-it law means that if you have not incurred enough expenses to reimburse the funds in your account at the end of the year, your remaining account balance will be forfeited. You do, however, have until March 31st annually to file your claims.
Keep your receipts! If you are audited, you’ll need proof that withdrawals were for qualified eligible expenses.

Life and Disability Insurance

Be prepared for the unexpected. Life and disability insurance can help provide for your loved ones if something where to happen to you.
Minnesota Masonic provides eligible employees with basic life and AD&D insurance for free, plus options to purchase additional voluntary life and long-term disability coverage at group rates. Life benefits are paid to your beneficiary upon death, while AD&D benefits cover specific injuries or accidental death.
Basic Life and AD&D
This is an employer-paid coverage offering financial protection in the event of death or serious injury due to an accident.
The life benefit is payable to your designated beneficiary in the event of your death equal to 1x annual base salary up to a maximum of $50,000. AD&D benefits are payable to you in the event of a dismemberment, or an additional benefit is payable to your beneficiary if your death is the result of an accident.
Voluntary Life and AD&D
You have the option to choose extra coverage for yourself and your family. You can select coverage in increments, reaching up to seven times your annual salary or a maximum of $200,000 for yourself, up to 3-1/2 x employee basic annual earnings to a maximum of $50,000 for your spouse and up to $10,000 for children over 6 months (to age 19, 25 years if a full-time student)
This provides additional financial support for your family in the event of your passing or if you are diagnosed with a terminal illness.
Voluntary Long-Term Disability
Offers additional protection beyond the employer-paid LTD plan, which offers a weekly benefit if you experience a covered disability that prevents you from working. Long Term Disability insurance can help replace a portion of your income during your recovery. Benefit feature include
What will my beneficiary receive?
In the event that death occurs: Your Basic Life insurance is paid to your beneficiary. If death occurs from an accident: 100% of the AD&D benefit would be payable to your beneficiary(ies) in addition to your Basic Life insurance.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
