Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by [CLIENT NAME]. When you enroll in the benefits program, you may also cover your eligible benefits. Dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.

Overview

As you know, [CLIENT NAME] sponsors a self-funded medical plan. This means you share in the cost of the plan through your payroll deductions, copays and coinsurance, [TPA] provides the plan administration and [CLIENT NAME] pays the claims. This year our plan ran well, meaning costs were well managed resulting in a good renewal. Some years we aren't so fortunate and increased costs must be shared. So given the personal and business challenges we have all experienced during the pandemic, we are happy to share this good news!

The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.


You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

LIFE & DISABILITY

[CLIENT NAME] provides you with Basic & Voluntary Life/AD&D coverage as well as Short- and Long-Term Disability through [CARRIER NAME].


Basic Life/AD&D

Employees working 30 or more hours per week are eligible for [$XXX] coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.


Voluntary Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Your coverage through [CARRIER] pays [XX%] of your income to a maximum of [$XXX] per week.


Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits begin after a [XX] day elimination period are are equal to [XX%] of covered monthly earnings up to [$XXX] for full-time employees.

FSA & HSA

The Health Care and Dependent Care Flexible Spending Accounts (FSA) and the Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to an FSA or HSA you reduce your taxable income, so you pay less in taxes — which saves you money.


HEALTH CARE FSA

  • You may contribute up to $2,750 annually
  • All benefit eligible can participate, unless you or your spouse are contributing to an HSA
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses


DEPENDENT CARE FSA

  • You may contribute up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns
  • Eligible expenses include care for your dependent child, spouse, parent or grandparent who are incapable of caring for themselves


FSA Contributions

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.


How the FSA Plan Works

You must incur your eligible expenses during the plan year — January 1 to December 31. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service. You do, however, have until March 31st annually to file your claims. Any funds after March 31 unclaimed will be forfeited.


HEALTH SAVINGS ACCOUNT (HSA)

  • You may contribute up to $3,650 annually as an individual or $7,300 as a family
  • You must be enrolled in a qualified HSA-eligible health plan, but you or your spouse may not participate in an FSA or Health Reimbursement Account (HRA)


How the HSA Plan Works

You may use your HSA funds to pay for eligible medical, prescription drug, dental or vision expenses. Unused funds remain in your account for future use and roll over each calendar year. You can change your HSA contribution at any time during the plan year for any reason.
