We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


Terry Barr Sales remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees. Our plans will remain the same for the 2023 benefit year.

NEW HIRE ENROLLMENT AND ELIGIBILITY

Eligibility

All full-time employees working at least 40 hours per week are eligible to participate in the benefit plans offered by Terry Barr Sales. Newly hired employees will be eligible the 1st of the month following 30 days of full-time employment. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.


Children are eligible for benefits as follows:


  • Medical and Dental: Until the last day of the year in which the child turns 26

Enrollment

ENROLLMENT INSTRUCTIONS:

  • Go to TerryBarrSales.ease.com
  • Log in to Ease per the instructions you have received from your HR administrator.
  • Follow instructions and enroll in your benefits
  • Make sure to save your elections and print your confirmation statement.


The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Life Events." These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a Qualifying Life Event, you may make changes to your benefits by contacting Human Resources and completing any election changes online within 30 days of the event.

PAYROLL LOGIN INSTRUCTIONS:

  • Paycor Secure Access Employee Login
  • Employees will receive an email from Paycor to set up a login and username. Once registered, you will be able to set up direct deposit and complete your W-4 and applicable withholdings.

Simply Blue PPO 250

The Simply Blue PPO option offers the freedom to see any provider when you need care. When you use providers from within the PPO network, you receive benefits at the discounted network cost. Most expenses, such as office visits, emergency room and prescription drugs are covered by a copay. Other expenses are subject to a deductible and coinsurance.

Simply Blue PPO HSA 1500

The Simply Blue PPO HSA Plan is a qualified high deductible health plan (HDHP). It is similar to the PPO Plan in that you have the option to choose any provider when you need care. However, you must satisfy a higher deductible that applies to almost all health care expenses, including those for prescription drugs. All expenses are your responsibility until the deductible is reached, with the exception of preventive care, which is covered at 100% when you visit a physician in the network. Once the deductible is met, you are responsible for coinsurance for medical expenses and a copay for prescription drug expenses.


Enrolling in this plan allows you to contribute tax free dollars to a health savings account (HSA). Any dollars that you (and your employer) wish to contribute can be used towards any eligible medical, Rx, dental and vision expenses that you may incur while covered under the plan.

Terry Barr Sales provides those employees who are covered by one of the medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.

If you enroll in the Simply Blue PPO HSA option, the Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money. I

To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:

  • You must be covered by a Qualified High Deductible Health Plan
  • You cannot be covered by another health plan (including if your spouse has a health care Flexible Spending Account)
  • You cannot be covered by Medicare or Medicaid
  • You cannot be eligible to be claimed as a tax dependent
  • You must have not received VA benefits in the past 90 days


HSA IRS Maximums for 2023 are as follows:

  • $3,850 for a single contract
  • $7,750 for a two person or family contract
  • $1,000 "Catch Up" contribution for those age 55+

Life and AD&D

You are automatically enrolled in Basic Life and AD&D insurance benefits. Your Basic Life Insurance benefit is 1x your base annual earnings, up to a $100,000 maximum (reduced if you are age 70 or older), and the amount of your AD&D benefit is equal to your Life insurance. You do not need to enroll in these benefits as they are provided at no cost to you.

Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Your Long Term Disability benefit is equal to 60% of your base monthly earnings up to a maximum benefit of $10,000 per month. You do not need to enroll in these benefits as they are provided at no cost to you. Serious illnesses or accidents can come out of nowhere. They can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits begin following a 180 day elimination period.
