AVA is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.



ENROLLMENT

The Benefit choices you make during your your open enrollment remain in effect for the entire year.


You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

MEDICAL/RX

Advanced Vehicle Assemblies – Tubular Metal Systems offers the Blue Care Network 1000 plan. This is a traditional plan with flat dollar copays for specific office services and prescription drugs. Preventive Care Services are covered at 100% and copays and deductibles do not apply. 


You must indicate your Primary Care Provider (PCP) with their provider codes on your enrollment form, for all members covered under the plan. If you do not select a PCP, one will be chosen for you based on your address. If you do get auto-assigned a PCP you can change your PCP through your member account or by calling the phone number on the back of your BCN ID card.

AVA – Tubular Metal Systems provides those employees who are covered by the medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.

DENTAL

Dental Plan

It’s important to have regular dental exams and cleanings to detect any issues before they become painful – and expensive. We offer dental coverage through Delta Dental.


Under this plan you have the option to choose any provider, but you will save more money when you see an in-network provider. You may pay more if you see an out-of-network provider because they have not agreed to negotiated rates.

VISION

BCBSM Vision Plan

Our vision plan provides coverage for routine eye exams ad pays for all or a portion of the cost of glasses or contact lenses. You can choose any provider; however, you always save money when if you see an in-network provider. Our Blue Vision benefits are provided through Vision Service Plan (VSP).

LIFE & DISABILITY

Unum

AVA – Tubular Metal Systems provides you with financial protection options for life, accidental death & dismemberment and short term disability and long term disability with Unum.


Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.
