Eligibility Information
Employees of Austin Couch Potatoes and their eligible dependents may participate in various employer-sponsored health and benefit programs as outlined in this benefit guide. Company employees are defined as:
Full-Time: Employees whose normal work week averages 30 hours or more, excluding lunch hours. You become eligible and may enter the plan(s) on the first of the month following a 30 day waiting period.

Download the My Benefits Work mobile app from the App Store or Google™ play.
2. Register
Register your account on the app or the web portal –you only need to register once and your login will work for both.
3. Enter ID
Use your Group ID and Member ID from the front of your membership card.
4. Enter Email
Enter your email address – this will be your username for the app and portal.
5. Enter Phone Number
Enter your mobile phone number and a password, then proceed.
6. Confirm
You will be prompted to confirm your account through text message or email. Enter the code and continue.

All employees should complete an election form to indicate whether they are enrolling or waiving Sedera (cost sharing plan), ICHRA, dental, and vision.
Anyone enrolling with Sedera must complete their online enrollment with Sedera by the 19th of the month in order to get a 1st of the month effective date. Sedera cannot backdate enrollments.
