Annual Enrollment

IF YOU WISH TO ENROLL IN NEW COVERAGE YOU MUST ACTIVELY MAKE AN ELECTION FROM APRIL 10TH TO APRIL 21ST. IF YOU ARE CURRENTLY ENROLLED IN BENEFITS AND YOU DON'T MAKE NEW ELECTIONS, YOUR CURRENT COVERAGE WILL ROLL OVER.


The 2023-2024 Open Enrollment period begins Monday April 10th and ends on Friday April 21st. This is the one time during the year to review your personal situation and determine whether to make changes to your current elections. You may also use this opportunity to enroll yourself and/or dependents if you waived coverage in the past. Outside of this enrollment period, you may only make benefit changes if you experience a qualified life event (e.g., birth, adoption, marriage, divorce, loss of spouse/partner health coverage). Enrollment Forms for 2023-2024 must be submitted to Human Resources by Friday April 21st.

ALLSTATE (MERITAIN) is our vendor for the Medical Program


Allstate Medical

  • Aetna Choice POS II Network
  • Two plans to choose from
  • 3-Tier Prescription drug coverage through the Cigna network
  • Prescription Drug Coverage and Preferred Drug List – with a large national network of chains, independent retail pharmacies and home delivery, and online tools to help you compare and save
  • Health and Wellness Resources – online and telephonic resources to help you get, and stay healthy
  • Customer Service
  • MERITAIN - you can contact Meritain by phone (1-800-925-2272), online at www.meritain.com, whenever you need assistance
  • CIGNA - you can download the myCigna mobile app from the App Store or Google play, or visit Cigna online at www.mycigna.com whenever you need assistance



How to Search for an In-Network Medical Provider

  1. Go to www.meritain.com
  2. Click on “ABC" under Provider network finder
  3. Select "Aetna”
  4. Enter your location and click "Search"
  5. Select "Aetna Choice POS II (Open Access)" under Broad Medical Networks and click "Continue"


How to Search for an In-Network Pharmacy

  1. Go to www.cigna.com
  2. Click on “Find A Doctor" next to "For Individual & Families"
  3. Select "Employer or School”
  4. Enter Address, City, or Zip
  5. Select "Health Facilities”
  6. Enter “Pharmacy” in the Search field
  7. You may Continue as a Guest or Log In if you already have a Cigna login
  8. Click on Continue without a plan




United Healthcare (UHC) will continue to administer the Dental and Vision Plans


Dental DPPO

  • Options PPO 20 Network
  • Annual preventative services covered at 100% in network and 80% out of network, not subject to the annual deductible

Vision Plan

  • The vision plan is through UHC's Spectera Network

How To Enroll

How to Enroll for the 2023-2024 Plan Year
  • Download the Enrollment Forms to get started. You must complete the Mattos Hospitality Enrollment Form as well as an Allstate (medical) Enrollment Form and/or UHC (dental and/or vision) Enrollment Form if you are enrolling for the first time, adding/dropping any dependents, or now waiving coverage. Be sure to return the form to Human Resources by April 21st in order to enroll in benefits.
  • Complete each step in your Open Enrollment. Make sure to actively elect or wave each benefit
  • All elections must be made by 11:59 PM EST on April 21, 2023
