AppSumo is pleased to offer a [NEW] selection of benefits designed with your health and financial wellbeing in mind. We have reviewed our benefit programs this year ensuring we have considered the best combination of benefit coverage, network access, and affordability for our employees.


Eligibility

If you’re a full-time team member at AppSumo, you’re eligible to enroll in the benefits outlined through your Connect Landing Page. Full-time team members are those who work 30 or more hours per week. In addition, the following family members are eligible for medical, dental and vision coverage:


  • Legal Spouse and Domestic Partners
  • Natural and Adopted Children, Stepchildren or any other children you support for whom you are the legal guardian or for whom you are required to provide coverage due to a qualified medical child support order

Enrollment

How To Enroll In Benefits

The decisions you make during open enrollment can have a significant impact on your life and finances, so it is important to weigh your options carefully. 


OPEN ENROLLMENT BEGINS August 1st - 10th

[New Benefits go into effect September 1, 2022 - August 31, 2023]


All Teammates are required to log into Insperity and complete their open enrollment. Once you have made your elections and your enrollment window has closed, you will not be able to make changes to your elections, until the next open enrollment period unless you have a qualified change in status.


Newly Hired Teammates: You will be eligible for benefits on the 1st day of employment. The benefits you elect will stay in effect until August 31, 2023.


HOW TO MAKE CHANGES


Unless you experience a life-changing qualifying event, you cannot make changes to your benefits until the next open enrollment period. Qualifying events include things like:


● Marriage, divorce or legal separation

● Change in number of dependents (includes birth, adoption and death)

● Loss of dependent status

● Change in employment status or change of coverage under another employer’s plan

● Judgments, orders or decrees

● Enrolling in or losing coverage under Medicare, Medicaid or CHIP

● While on leave under FMLA


Qualifying Life Event Changes must be made within 30 days of the event date by contacting your People Team.

DENTAL

BCBS Dental Plan

NEW! We have moved to BCBS with ONE similar plan option but with enhanced benefits!


BlueCross and BlueShield of Texas

You have the freedom to select the dentist of your choice; however, when you visit a participating in-network dentist, you will have lower out-of-pocket costs, no balance billing, and claims will be submitted by your dentist on your behalf.


PRE-TREATMENT ESTIMATE

If your dental care is extensive and you want to plan ahead for the cost, you can ask your dentist to submit a pre-treatment estimate. While it is not a guarantee of payment, a pre-treatment estimate can help you predict your out-of-pocket costs.


How do I find an In-Network Provider?

This dental plan offers deeper discounts when you visit a provider that is In-Network. In-Network providers can be found on www.bcbstx.com under “Find Care”. Both dental plans utilize the BlueCare Dental Network.

DID YOU KNOW?

  • Eye exams can help detect serious eye and general health conditions sooner: high blood pressure, diabetes, heart disease, high cholesterol
  • Babies should receive their first professional eye exam at 6 months
  • 80% of learning in the first 12 years comes through the eyes


Under this plan, you may use the eye care professional of your choice. However, when you visit a participating in-network provider, you receive higher levels of coverage. If you choose to receive services from an out-of-network provider, you will be required to pay that provider at the time of service and submit a claim form for reimbursement.

LIFE & DISABILITY

Guardian

New Waterloo provides you with Basic & Voluntary Life/AD&D coverage as well as Voluntary Short-Term Disability and company paid Long-Term Disability through Guardian.


Basic Life/AD&D

Eligible employees are eligible for $25,000 of coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.


Voluntary Life/AD&D

New Waterloo offers eligible employees the opportunity to purchase additional voluntary life and AD&D insurance for themselves and their dependents at discounted group rates. Term Life is an affordable way to cover yourself and your family for life insurance.


  • Employee: In units of $10,000, up to $500,000. Guaranteed Issue* up to $130,000. Proof of good health (Evidence of Insurability) will be required on any amounts above $130,000.
  • Spouse: Up to 100% of the employee’s term life amount , in units of $5,000, up to $250,000. Guaranteed Issue* up to $50,000. Proof of good health (Evidence of Insurability) will be required on any amounts above $50,000.
  • Children: $1,000, $5,000 or $10,000 not to exceed 100% of employee’s term life amount.


A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Voluntary Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Your coverage through Guardian offers:


Employee optional coverage

You can elect to purchase a benefit of 60% of your

weekly earnings to a maximum of $1,000 whichever is less.

Coverage begins after a 14 day waiting period and continues for up to 11 weeks.


Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits begin after a 90 day elimination period are are equal to 60% of covered monthly earnings up to $6,000 whichever is less for eligible employees.

HSA

Insperity

HEALTH SAVINGS ACCOUNT (HSA)

  • You may contribute up to $3,650 annually as an individual or $7,300 as a family
  • You must be enrolled in a qualified HSA-eligible health plan, the Base: HDHP (High Deductible Health Plan)


How the HSA Plan Works

You may use your HSA funds to pay for eligible medical, prescription drug, dental or vision expenses. Unused funds remain in your account for future use and roll over each calendar year. You can change your HSA contribution at any time during the plan year for any reason.

What’s next?


Watch for an email from customersuccess@nayya.com or your HR department at the start of your benefits open enrollment. It will contain your personalized link to the Nayya platform. Follow the directions to register, and you’ll then be guided to answer a few questions. Once you’re finished, you will be provided with recommendations and be directed to your workplace enrollment website to make your benefit elections.
