Helping you and your family reach and maintain good health is very important to STARRY.

STARRY is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind.

We review our benefit programs each year to ensure we consider the best combination of benefit coverage,

network access and affordability for employees.


Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by STARRY. When you enroll in the benefits program, you may also cover your eligible benefits. Dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.

OVERVIEW

STARRY offers benefit plan options through Blue Cross Blue Shield of Texas

Two Medical plan options:

Base Plan – MTBCBB024 - $2,500 deductible – Blue Choice PPO Network

Buy-Up Plan – MTBCB014 - $1,500 deductible – Blue Choice PPO Network


Two Dental plan options:

Base Plan – Blue Care Dental DTNLM38 with a $1,500 annual maximum benefit per Member

Buy Up Plan – Blue Care Dental DTNHR31 with a $3,000 annual maximum benefit per Member


Vision Coverage


Employer paid Life Insurance and Voluntary Supplemental Life Insurance


Employer paid Short and Long Term Disability coverage

ENROLLMENT


Choose your Benefits carefully.

The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire plan year.

You can, however, modify your elections under certain circumstances, called "Qualifying Events".

These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan.

If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.



Open Enrollment - All Currently Enrolled Employees:

You will use the same login and password information you used to complete your initial enrollment through the EASE portal.

If you have forgotten your password, you will be able to re-set it.


How to Enroll in Benefits

All team members have access to our Ease online benefits enrollment platform 24/7 where you have the ability to enroll, select, or change your benefits online during the annual open enrollment period, new hire enrollment period, and for qualifying events. Accessible 24/7;

  • Make changes to beneficiary designations and more.

There are two medical plan options through BCBS to choose from.

Both options are PPO plans


PPO | In-Network & Out-of-Network Benefits Available

PPO plans offer you the freedom to see any provider when you need care.

•When you use providers from within the PPO network, you receive benefits at the discounted network cost.

•Most expenses, such as office visits, emergency room and prescription drugs are covered by a copay. Other expenses are subject to a deductible and coinsurance.

You may select the dentist of your choice; however, when you visit a participating in-network dentist

you could have lower out-of-pocket costs, no balance billing, and claims will be submitted by your dentist on your behalf.

EyeMed Vision care network

The Vision plan assists with copayments for eye exams, prescription glasses and contact lenses.

You receive a higher level of benefits when you see a provider in the EyeMed Network.

Proficient Benefits

Flexible Spending Accounts –

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.

Contributions

You must make a new election each year! The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.

You may contribute as follows:


Health Care FSA -

Up to $2,850 annually with up to a $550 roll over amount if funds are not used.

All benefit eligible can participate, unless you or your spouse are contributing to an HSA.

Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses.


Dependent Care FSA

Up to $5,000 annually

Limited to $2,500 if you are married and file separate tax returns

How the Plan Works

The person whose expenses you are claiming must be

 • your qualifying child under the age of 13, who shares the same residence with you; or

• your spouse or qualifying child or qualifying relative who is physically or mentally unable to

care for him/herself who shares the same residence with you and has income less than the Federal exemption amount.

You must incur your eligible expenses during the plan year. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service. You do, however, have until March 31st annually to file your claims. Any funds after March 31 unclaimed will be forfeited.


LIFE / AD&D

STARRY provides all eligible employees with Basic Life/AD&D coverage at no cost.


Basic Life/AD&D

Employees working 30 or more hours per week are eligible for Basic Life coverage of 2x your annual earnings payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.

 

Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child. You Life Beneficiary information can be updated at anytime through the EASE Portal.


Supplemental Life

In addition to your employer provided Basic Life insurance coverage, you have the opportunity to enroll in voluntary/optional/supplemental Life insurance coverage. Coverage is also available for your eligible dependents, however, It is required that you elect coverage for yourself in order to elect coverage for your dependents. 


*Guaranteed Issue (GI) and Evidence of Insurability (EOI)

When you are first eligible (at hire) for Voluntary Life and AD&D, you may purchase up to the Guaranteed Issue (GI) for yourself and your spouse without providing proof of good health (EOI).

Any amount elected over the GI will require EOI. If you elect optional life coverage and are required to complete an EOI, it is your responsibility to complete the EOI and make sure it gets to the provider or your HR department for processing with the Provider. Elected coverage over the GI amount will not be installed until the EOI is received and the carrier has approved it. In addition, your spouse will need to provide EOI to be eligible for coverage amounts over GI, or if coverage is requested at a later date.


At Open Enrollment:

Employees currently enrolled in Vol Supplemental Life can increase their coverage by one $10k increment up to the Guaranteed Issue amount without EOI.

For example, if an Employee has $30k, they could increase to $40k at annual enrollment without EOI.

EOI is required for any spouse increase.

EOI is not required for children.

Any employee who previously declined Supplement Life coverage and wants to add the coverage at Open Enrollment, EOI is required for any EE or SP elections. 


DISABILITY

Blue Cross Blue Shield of Texas

STARRY provides Disability Income Protection to all eligible Employees at NO COST.


Short-Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Your coverage pays 60% of your income to a maximum of $1,500 per week.


Long Term Disability

Disability benefits are designed to replace a portion of your income if you are unable to work for an extended period of time due to an accident or sickness. Benefits begin after a 90 day elimination period and are equal to 60% of covered monthly earnings up to $7,500 for all eligible employees. The benefit paid to you is taxable.
