Our Medical benefit plans are staying the same and there are no increases to your premiums.

 

Smylie One works diligently to provide you with an affordable and best in class Health Care Program.

 

Annual enrollment is your opportunity to learn about the 2022 Health Care Program, review your current coverage in Ease and choose the best options for you and your family.

 

Remember, this is the only time during the year that you can enroll in coverage or change your benefit elections, unless you have a qualifying event during the year. You have 30 days from the qualifying event to notify HR to make the changes. Common life events are; marriage, divorce, birth, adoption and loss in coverage.

How to enroll or make changes to medical with Anthem

ENROLLMENT INSTRUCTIONS:

1. You will receive an Email with a link to Ease. Click the

blue “login” button with the email message to be directed to the

enrollment portal.

2. Enter your User ID and password

1. If you do not remember your User ID and Password please

contact Naomi Butze with NFP at Naomi.butze@nfp.com.

3. Follow instructions and enroll in your benefits

4. Make sure to sign your forms, save your elections and

print your confirmation statement.
