- If you want to take part in a Flexible Spending Account, you need to make an active election each year. Previous elections do not carry over.
Benefits Eligibility and Qualifying Events
City of Sand Springs Benefits Program
Eligibility
All full-time employees are eligible to participate in benefits offered by the City of Sand Springs. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include:
- Your legal spouse
- Children up to age 26 (regardless of student status, marital status, residence, or financial dependence on you)
- An unmarried disabled child incapable of self-support
Qualifying Events
A Qualifying Life Event (QLE) is a significant change in a person's life that allows them to make changes to their health insurance outside of the regular open enrollment period. Examples of qualifying life events include:
- Marriage
- Divorce
- The birth of a child
- Loss of health coverage
- A change in employment status
You must notify HR within 30 days of a QLE to make a change to benefits. If you miss this window, you may have to wait until the next open enrollment to make your updates or changes to benefits.

Health Maintenance Organization (HMO) Plans
An HMO plan is a type of health insurance that works through the HMO Select network of doctors and healthcare providers. With an HMO, you will need to choose a HMO Select participating primary care physician (PCP). Your PCP will be your go-to healthcare professional, guiding and coordinating all your care.

Overview of Benefit Resources, Inc.
Flexible Spending Accounts
Health Care and Dependent Care Flexible Spending Accounts (FSA)
Benefit Resources, Inc. (BRI) offers two types of Flexible Spending Accounts (FSAs): Health Care FSA and Dependent Care FSA. These accounts allow you to set aside pre-tax dollars to pay for eligible expenses, reducing your taxable income and saving you money. Keep in mind that the IRS requires new elections each year during open enrollment.
Contributions
The election you make during open enrollment remains your election for the entire plan year, with payroll deductions occurring throughout the year. Changes to your election can only be made if you experience a qualifying life event, and the request must be consistent with that event.
Health Care FSA
- Maximum annual contribution: $3,300
- Full election amounts available on July 1
- Unused balances over $660 at the end of the plan year will be forfeited
- Eligibility: All benefit-eligible employees, except those or their spouses contributing to an HSA
- Funds can be used for out-of-pocket healthcare costs (medical, dental, vision, hearing, prescriptions, and certain over-the-counter items)
Dependent Care FSA
- Maximum annual contribution: $5,000 (or $2,500 if married and filing separate tax returns)
- Funds are available only once deposited into the FSA
- Funds can be used to pay for out-of-pocket childcare expenses while you are working, for children under 13 years old
- Unused balances cannot be rolled over and will be forfeited
