This is your opportunity to learn about the Saga Communications benefit plan offerings. Please review your 2022 Benefit Guide to see what plans are available with Saga Communications. Once your enrollment window has closed, you will not have the opportunity to make changes to your elections until open enrollment, unless you have a Qualifying Life Event.


Once you have reviewed your benefit information and are ready to submit your elections, please visit www.workforcenow.adp.com. Your elections must be submitted within 14 days of employment and benefits will begin on the 91st day of active employment.

ENROLLMENT INSTRUCTIONS:

1. Review the information in this guide and benefit plan summaries.

2. You must login to ADP www.workforcenow.adp.com and enroll or waive in coverages.

3. You will not be allowed to make changes after the open enrollment window closes, unless you experience a qualifying life event.



The Benefit choices you make during your open enrollment remain in effect for the entire year.


You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.


To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:

  • Covered by a Qualified High Deductible Health Plan
  • You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
  • You cannot be covered by Medicare or Medicaid
  • You cannot be eligible to be claimed as a tax dependent
  • You have not received VA benefits in the past 90 days


HSA IRS Maximums are as follows:

$3,650 for a single contract

$7,300 for a two person or family

Saga Communications provides those employees who are covered by one of the BCBSM medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.


How Much Does it Cost?

  • PPO 1000/PPO 2000: $20 copay
  • PPO HSA 3000: Covered-80% after deductible
  • Prescriptions: If prescribed during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in.

Saga Communications dental plans provide comprehensive coverage to help you and your family maintain good dental health. For 2022, the dental carrier will be Delta Dental.

Eligible employees may pick between the Basic or Enhanced plans. The Enhanced plan will provide richer benefits, orthodontia coverage, as well as both Delta Dental PPO and Premier dentists being considered as in-network dentists. The Base plan is limited to include Delta Dental PPO dentists as in-network.

Saga Communications offers vision coverage through VSP to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. Saga Communications provides you with financial protection options for life, accidental death & dismemberment with Lincoln Financial Group.


Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


  • Basic Life/AD&D Employer Provided Benefit - No Cost to You
  • Flat $40,000
  • Spouse (up to $4,000) Child(ren) (up to $2,000) Max benefit for dependents 14 days to 6 months is $1,000
  • Voluntary Benefit - Costs are based on Age and Amount of Coverage
  • Employee: Option 1: 1X Basic Annual Earnings; Option 2: 2X Basic Annual Earnings; Option 3: 3X Basic Annual Earnings; Benefit Maximum: $750,000 - Guaranteed Issue: $450,000
  • Spouse and Child: Option 1: Spouse (up to $8,000) Child (up to ($4,000) Option 2: Spouse (up to $12,000) Child (up to $6,000) Option 3: Spouse (up to $20,000) Child (up to $10,000)


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

Short Term Disability

Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.


  • STD Employer Provided Benefit - No Cost to You (Employees with more than 6 months service at open enrollment)
  • 60% of Base Weekly Earnings up to a weekly maximum of $250 (Benefits begin on the 8th day for Accident / 8th day for Illness)
  • STD Voluntary Benefit
  • Option 1: 60% of Base Weekly Earnings up to a weekly maximum of $250 (Benefits begin on the 8th day for Accident / 8th day for Illness)

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.


New for 2022 - those who elect the Dependent Care FSA will now receive a debit card.


Contributions

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.


You may contribute as follows:

  • Up to $2,750 annually
  • All benefit eligible can participate
  • If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses

  • Limited to $2,500 if you are married and file separate tax returns
