Benefits Open Enrollment

Open Enrollment is your opportunity to evaluate your current benefit elections and make changes for the new plan year. You are given an Open Enrollment opportunity each year for a June 1 renewal.

Please review the Open Enrollment Overview for enrollment instructions. 

Open Enrollment paperwork is due to HR by May 23rd.  

Changes This Year

The Housing Authority is committed to offering a comprehensive employee benefits program that helps our employees stay healthy, feel secure and maintain a work-life balance. 

Medical Benefits: Our medical plans incurred a significant increase this year, so we’re changing the network for our Silver and Gold plans to Blue Advantage. Blue Advantage is a smaller network but comes with lower premiums due to deeper negotiated discounts. This means less out of pocket for you, with no change in benefits.

Your Cost Per Paycheck: Our medical premiums are increasing, so you’ll see a change to your cost per pay for this benefit. No other benefit incurred a rate increase; however, you may see an increase to your Voluntary Life and Critical Illness payroll deductions if you advanced to a new age band.

Premium Subsidy Program

AFFORDABLE HEALTH INSURANCE FOR EMPLOYEES IN OKLAHOMA

Health insurance can be expensive, even if you and your employer split the cost. Insure Oklahoma keeps businesses and working adults strong by helping them pay for health coverage. The Employer-Sponsored Insurance program can save you 60 percent or more on health insurance premiums.

Through this state-operated program, premiums for Sac & Fox's Platinum plan are split between the casino and Insure Oklahoma (60 percent or more). Insure Oklahoma’s share is paid with state tobacco tax and federal matching dollars.

You may be able to get assistance in paying premiums for qualified family members, too.

HOW TO QUALIFY

To qualify for Insure Oklahoma’s Employer-Sponsored Insurance program, you must work for a business that has already been approved by Insure Oklahoma, and:

  • Be between the ages of 19 to 64
  • Be an Oklahoma resident and meet citizenship guidelines
  • Have an annual gross household income within the Employer-Sponsored Insurance income guidelines
  • Not be enrolled in Medicare or Medicaid
  • Contribute up to 15 percent of monthly premium cost for yourself, and up to 15 percent of premium cost for qualified dependents (but the total premium won’t be more than 3 percent of your annual gross household income).

HOW IT WORKS

Insure Oklahoma sends its share of the premium to your employer every month. Your employer deducts your 15% share from your paycheck and combines those shares with the employer's to pay the insurance company. You can use your health plan’s benefits and network of doctors to stay healthy.

Quick Reminder

If you were previously approved for Insure Oklahoma, you will need re-apply online on your one-year anniversary of approval. Go to www.insureoklahoma.org and click “Apply”. You must re-apply every year, or eligibility will end.

How To Enroll and Forms

Be sure to attend the open enrollment meeting to get your benefits packet.


For currently enrolled Employees, your benefits packet includes your personal Benefits Statement, which reflects your current benefit elections and payroll deductions. It also includes your personal benefits enrollment/change form. Please complete this form and return to HR by the open enrollment deadline. Note: Your benefits statement and enrollment form might not reflect any recent election changes.


For newly benefit eligible Employees, your packet includes a benefits application. Please complete this form and return to HR no later than your 60th day of employment.


ACTION REQUIRED

Everyone is required to complete a new benefits application. You can find your custom form in your benefits packet.

Anyone that thinks they may qualify for the Insure Oklahoma program, must apply online.


Your applications must be submitted to HR and online enrollments done by May 23rd.

Symetra

Life and Disability Benefits

As your employer, we automatically provide a certain level of life insurance coverage for you and give you the opportunity to purchase additional coverage for you and your dependents. 


Employees who wish to supplement their basic life insurance benefit, may purchase additional coverage.


Annual Voluntary Life Benefit Increase:

Employees that elected this coverage during their initial enrollment period, have the option to elect $10,000 to $50,000 (not exceed the guaranteed issue amount) additional coverage, during open enrollment, without having to prove insurability. This benefit is not available for your spouse or dependent children.


If you didn't elect this benefit, when first eligible, and decide to enroll during open enrollment, you will have to prove insurability by completing an online Evidence of Insurability (EOI). The link for this form can be found in the "How to Enroll and Forms" section. Underwriting will notify you of approval or denial of coverage.
