What' New for 2022!

United Healthcare is the NEW carrier for the Basic life Life/AD&D benefit and Voluntary Life/AD&D Benefit

  • NEW Company Paid Life/AD&D for all full-time employees - $25,000
  • NEW Carrier for Voluntary Life/AD&D for employees, spouse and dependents at lower premiums.
  • NOTE: Minimum participation MUST BE MET

ENROLLMENT INSTRUCTIONS:


You will receive an Email with a link to Ease. Click the blue “login” button with the email message to be directed to the enrollment portal.

1.Enter your User ID and password

If you do not remember your User ID and Password please contact Kelli Krise with NFP at Kelli.Krise@nfp.com.

2.Follow instructions and enroll in your benefits

3.Make sure to sign your forms, save your elections and print your confirmation statement.

Medical/RX: 3 new plan designs provided through United Healthcare

  • Plan 1* - 70% HMO: $6,000/$12,000 ded NHP network. (PCP)
  • Plan 2* - 80% HSA HMO: $5,000/$10,000 ded NHP network. (PCP)
  • Plan 3 - 80% PPO: $1,000/$2,000 ded Choice Plus Network.

Notes: Plans 1 & 2 require designation of a Primary Care Physician (PCP) who participates in Neighborhood Health Partnership Network (NHP). No referrals are required for specialists.

  • Plan 2 includes the Motion Rewards Program where you can earn up to $1,095 per year towards your HSA.

  • Plan 1 - DHMO: Schedule of benefits only. Primary DDS not required.
  • Plan 2 - PPO Options 30: $50/$150 deductible; 100%/90%/60%/50%
  • In-Network. Non-Network reimbursement at 90th UCR.

Real Res offers vision coverage through United healthcare who utilizes Spectera network – no changes.to help pay for eye exams, prescription glasses and contact lenses. AFLAC enrollment through EASE, payroll deducted on a post tax basis.


Aflac Product Offerings are available to all full-time employees (not tied to medical enrollment). Cash Benefits paid directly to the member.

Pre-Existing Condition Limitations

  • Cancer – must be cancer-free for 5 years
  • Critical Care – no heart attack, stents, stroke or bypass surgery in the last 12 months.


Accident provides a well benefit of $60 for wellness services (NOTE: wellness is provided @ 100% under medical plan. Can submit bill to AFLAC and receive $60 reimbursement.)  If you leave employment, your policy can be continued on a direct basis with AFLAC.
