You are eligible to participate if you are full-time and work a minimum of 40 hours per week. Your coverage will be effective 1st of the month following 60 days from your date of hire. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.
Children are eligible for Medical and Dental benefits until the last day of the year in which the child turns 26.
ENROLLMENT INSTRUCTIONS:
The Benefit choices you make now will remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Life Events." These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a Qualifying Life Event, you may make changes to your benefits by contacting Human Resources and completing any election changes online within 30 days of the event.

Preventative care services are covered 100% in-network. Preventative services included women's preventative healthcare services, well-child visits, flu shots and routine physical exams.
An HMO gives you access to doctors and hospitals within the HMO network. But unlike PPO plans, care under an HMO plan is covered only if you see a provider within that HMO’s network. To enroll in the HMO, you must live in Michigan and select a primary care physician (PCP), and you must obtain a referral from your PCP for other care. Quasar Industries offers two different HMO options.
The BCBSM Community Blue 250 option is a PPO, which offers the freedom to see any provider when you need care. When you use providers from within the PPO network, you receive benefits at the discounted network cost. Most expenses, such as office visits, emergency room and prescription drugs are covered by a copay. Other expenses are subject to a deductible and coinsurance.

Quasar Industries offers both Health Care and Dependent Care Flexible Spending Accounts (FSA) that allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.
Health Care FSA
Allows you to pay for medical, dental and vision expenses that are not covered by your plans with pre-tax dollars.
Dependent Care FSA
Allows you to pay for eligible child or elder care expenses with pre-tax dollars while parents are at work

You are automatically enrolled in Basic Life and AD&D insurance benefits. Your Basic Life Insurance benefit is 1 x your annual earnings, up to a maximum of $150,000 (reduced if you are age 70 or older). You do not need to enroll in these benefits as they are provided at no cost to you.
Accidental Death & Dismemberment (AD&D) Benefit doubles if the death occurs by an accident
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. The Short Term Disability coverage provides you 60% of your base weekly earnings (up to $400 per week) for up to 26 weeks. This benefit is provided at no additional cost to you. Benefits begin on the 1st day of an accident and 8th day of an illness.
