Summary of Benefit Changes Effective August 1, 2024:

• Medical Plan Changes - High Plan
• Increased the in-network Out-of-Pocket Maximum (Individual/Family) to $9,100/$18,200 from $8,950/$17,900
• Increased the out of network Out-of-Pocket Maximum (Individual/Family) to $18,200/$36,400 from $10,400/$20,800

• Medical Plan Changes - Mid Plan
• Increased the in-network Deductible (Individual/Family) to $3,200/$6,400 from $3,000/$6,000
• Increased the in-network Out-of-Pocket Maximum (Individual/Family) to $8,050/$16,100 from $7,400/$14,800
• Increased the Emergency Room to $500 after Deductible from $350 after Deductible
• Increased the out of network Deductible (Individual/Family) to $6,400/$12,800 from $6,000/$12,000
• Increased the out of network Out-of-Pocket Maximum (Individual/Family) to $16,100/$32,200 from $15,400/$30,800

• Medical Plan Changes - Low Plan
• Increased the in-network Out-of-Pocket Maximum (Individual/Family) to $8,050/$16,100 from $7,450/$14,900
• Increased the Emergency Room to $500 per occurrence + Deductible from $350 after Deductible
• No Copay’s after Deductible for Retail and Mail Order prescription drugs
• Increased the out of network Out-of-Pocket Maximum (Individual/Family) to $16,100 / $32,200 from $15,400 / $30,800
• Change the out of network Coinsurance to 20% from 30%
