Welcome

Helping you and your family reach and maintain good health is very important to Plus Relocation. Plus Relocation is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Plus Relocation. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse, domestic partner, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.

How To Enroll In Benefits

All elections must be submitted by 10:00 pm CT on December 2, 2022. The benefits you elect during open enrollment will be effective from January 1, 2023 to December 31, 2023.


You are required to actively enroll in benefits for 2023, even if you have no changes.


The benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.


You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, or loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Medical

Medica will continue to administer your medical and prescription drug benefits this year.


All options have been carefully selected to provide the most value to you and your family. Plus Relocation will continue to offer the 300-$25-25% plan, the 1500-0% HSA plan, and the 3000-0% HSA plan. All plans utilize the Passport network.

Plus Relocation's dental plan provides comprehensive coverage to help you and your family maintain good dental health.


Plus Relocation pays 85% of the monthly dental premiums. Your total monthly premiums for our 2023 dental plans are provided below:


Plus Relocation will continue to offer the plan through Delta Dental that includes Orthodontics. Please refer to the benefits summary below to see plan details.

Vision

EyeMed Vision Plan

Did you know?

  • Eye exams can help detect serious eye and general health conditions sooner, such as: high blood pressure, diabetes, heart disease, and high cholesterol.
  • Eye Exams are covered through Medica medical insurance as preventative care.
  • Babies should receive their first professional eye exam at 6 months.
  • 80% of learning in the first 12 years comes through the eyes.


Plus Relocation offers vision coverage through EyeMed to help pay for prescription glasses and contact lenses. Plus Relocation has the Eye360 benefit in their vision plan which features an additional $50 added to your frame allowance at PLUS Providers. PLUS Providers are available nationwide in over 4,000 convenient, easy-to-find locations, including independent, retail and online options. Life & Disability

Unum provides you with Basic & Voluntary Life/AD&D coverage as well as Short- and Long-Term Disability.


Basic Life/AD&D

Employees working 30 or more hours per week are eligible for $25,000 coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.


Voluntary Life/AD&D

A sudden accident or death can leave you and your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance, which supplements lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Your coverage through Unum pays 60% of your income to a maximum of $3,600 per week.


Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits begin after a 90-day elimination period and are equal to 60% of covered monthly earnings up to $5,000 for full-time employees.

FSA

Flexible Spending Accounts- Medsurety

New third-party administrator for Flexible Spending Account (FSA) and Dependent Care Account (DCA)

Effective 1/1/2023 Medsurety will be replacing HR Simplified for the FSA and DCA


Enrollment for the FSA plans occurs during open enrollment for the calendar year. The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.


Contributions

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.


You may contribute as follows:

Traditional Health FSA

  • Up to $3,050 annually
  • Reimbursements for health, dental, and vision if not participating in the HSA benefit


Limited Scope FSA

  • Up to $3,050 annually
  • Reimbursements allowed for dental and vision expenses only
  • You can be enrolled in a HDHP


Dependent Care FSA

  • Up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax return
  • Covers eligible dependent care expenses (I.e., daycare expenses, before school or after school care for children 12 and younger, custodial care for dependent adults, licensed day care centers, nanny, etc.)


How the Plan Works

You must incur your eligible expenses during the plan year — January 1 to December 31. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service. You do, however, have until February 28th annually to file your claims. Any funds remaining unclaimed will be forfeited.


Submit a Claim

Below is a sample timeline of Medsurety's claim reimbursement process, detailing the events from the time you submit your claim until you are sent the reimbursement. Need to get your reimbursement even faster? Submit your claim online at www.medsurety.com. Medsurety prioritizes online claims submissions, always processing these requests first for the fastest turnaround time possible.

  • Day 1: Request reimbursements via mobile app, on-line portal, fax or mail
  • Day 1-3: Claim is reviewed, approved, entered into the system and posted
  • Day 4-5: Claim is processed for bank reimbursement file
  • Day 6-7: Reimbursement amount is deposited in employee checking account
  • Claims are processed on Wednesday to pay out either by direct deposit or check by mail Friday.
  • During high volume times throughout the year, claim timeline may be extended to no more than 10 business days.
  • Note: All days above reflect business days and direct deposit established in on-line account.

Health Savings Account - Medsurety

The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible expenses. By contributing to an HSA, you reduce your taxable income, so you pay less in taxes — which saves you money.


To be eligible for an HSA you:

  • Must be covered by a High Deductible health plan (HDHP)
  • Cannot be covered by other health coverage that is not an HDHP
  • Cannot be enrolled in Medicare, Medicaid or Tricare
  • Cannot be claimed as a dependent on another person’s tax returns


You may contribute as follows:

  • $3,850 for a single contract
  • $7,750 for a two person or family
  • $1,000 catch up if over the age of 55
