ENROLLMENT

Benefits begin on the first of the month following 60 days of employment.


Action Required:

  1. Go to https://workforcenow.adp.com
  2. Create an account Note: ADP can be reached at 855-693-2181 or by email at: hrservicesemployee@adp.com.
  3. Verify all of your personal information.
  4. After making your elections, you have the option to print out your Enrollment Summary Statement that contains all of your benefit elections and beneficiary designations.



Remember, this is the only time during the year that you can enroll in coverage or change your benefit elections, unless you have a Qualifying Life Event during the year. You have 30 days from the qualifying event date to notify HR to make the changes. Common life events are marriage, divorce, birth, adoption, and loss in coverage.

Priority Health will administer your medical and prescription drug benefits this year. To find out if your provider is in-network, please visit the website below. Your medical card will say Priority Health with a small Cigna logo on the back, but for claims, questions, and information about how a service might be paid, you will always call Priority Health, not Cigna.

ONLINE VISITS

MDLIVE


A virtual visit lets you see and talk to a doctor from your mobile device or computer. When you use one of the provider groups in our virtual visit network, you have benefit coverage for certain non-emergency medical conditions. Costs must be paid by you at the time of the virtual visit and will apply toward your deductible 

and out-of-pocket maximum.

Delta Dental of Michigan


Plum Market's dental plan provides comprehensive coverage to help you and your family maintain good dental health. Providers are part of the Delta Dental network, or you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your costs will be higher. BCBSM/VSP


Plum Market offers vision coverage through BCBSM/VSP to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in the VSP Network. Your vision card will say BCBSM this is who will pay the claims, but the network is VSP.

Plum Market provides you with Basic & Voluntary Life/AD&D coverage as well as Short- and Long-Term Disability through UNUM.


Basic Life/AD&D

Employees working 30 or more hours per week are eligible for a flat $25,000 coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.


Voluntary Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Voluntary Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.



Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

Short-Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. The voluntary 100% employee paid coverage through UNUM is highlighted below.

The “Be Well’ benefit pays you a lump sum of $50 per covered individual just for seeing your Doctor. Covered Wellness screenings include annual exams by a physician including physicals and well-child visits, and screenings for cancer, diabetes, cardiovascular health and more.

Life Planning - Financial and Legal Resource: With Life/AD&D, after a claim is submitted and approved, a specially trained consultant will reach out to the employee or beneficiary to provide support. Each consultant holds a Masters' dress in the mental health field and is highly skilled at assisting those who need helping to deal with the emotional challenges of terminal illness or loss of a loved one. They can also provide financial and legal support regarding estate settlement, Social Security, cash flow, taxes, and investment planning.
