OPEN ENROLLMENT
Annual enrollment is your opportunity to learn about your employee benefits, review your current coverage and choose the best options for you and your family.
UPMC Medical plans are effective January 1, 2026 through December 31, 2026.
There are two medical plans to choose from:
UPMC PPO $2,000 1X $20/$40 $8/$38/$76 RX (new plan for 2026)
UPMC PPO $3,500 $20/$40 $8/$38/$76 RX
United Concordia Dental plan is effective June 1, 2025 through May 31, 2026.
Highmark Vision plan is effective July 1, 2025 through June 30, 2026.
If you are a new employee, you have the opportunity to elect coverages:
Medical - effective 1st of the month following 60 days of employment - you have a max of 30 days from the effective date to make a decision.
Dental - effective 1st of the month following 90 days of employment - you have a max of 30 days from the effective date to make a decision.
Vision - effective 1st of the month following 90 days of employment - you have a max of 30 days from the effective date to make a decision.

- Mandatory Benefit Notices (Health Compliance Notices)
WHAT YOU NEED TO DO
If you are currently enrolled in the medical, dental or vision plan(s) and you decide to keep the same benefits you do nothing as your plans will roll over into the next plan year.
If you have been an employee and waived the coverages during your new hire enrollment period:
Open enrollment is your opportunity to elect coverages.
Please complete the appropriate application(s).
If you are currently enrolled in the medical, dental or vision plan and you need to make changes:
- Adding or dropping a dependent
- Change of address
- Changing medical plans
- Terminating your coverage
You will also need to complete the appropriate application(s).

Your completed forms should be sent to Amy Yarkowski NO later than 30 days from your effective date of coverage.
