Helping you and your family reach and maintain good health is very important to Patton Wings. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. Patton Wings remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees.

OPEN ENROLLMENT REMINDER!
Open Enrollment is from April 6, 2026 through April 20, 2026. This is the time of year where you can elect new plans with Patton Wings. You may switch between plans and make changes without being required to have a Qualifying Life Event. Please also keep in mind the following when making your elections for 2026:
- Evidence of Insurability is required if you are electing the Voluntary Life, STD, LTD (Prudential) and Critical Illness Insurance (Unum) for the first time during open enrollment.
- THIS IS A PASSIVE ENROLLMENT! If you do not make an active election by 4/20/2026, your current elections will carry forward with the exception of your HSA contributions.
- If you do not make an active election for your HSA, your HSA contributions will end of as 4/30/2026.
- HSA contributions do not require a Qualifying Life Event and can be started, changed, or cancelled at any time (payroll processing may require some lead time for updates to be reflected).
Eligibility & Enrollment
All full-time, salaried employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Patton Wings. Newly hired employees will be eligible the 1st of the month following 30 days of full-time employment. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and dependent children. Dependent Children are eligible for benefits until their 26th birthday.
Should your employment or eligibility end with Patton Wings, your benefits coverage will end as of the date of termination or ineligibility.
ENROLLMENT INSTRUCTIONS: Click the button below and log into your Alliance MyPay portal. Complete your elections through the portal by following the prompts.
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called Qualifying Life Events (QLE). These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event, or 60 days if event is adoption.

ASR will continue to be the administrator for your medical coverage and EHIM for your prescription coverage. You have the choice between three plans for the 2026-27 plan year. The three plan options are all PPO plans, two of which can be paired with a Health Savings Account (HSA). View the Providers & Networks section for more information as to how to locate participating providers.
If you are already enrolled in one of the Patton Wings ASR medical plans today and have not already done so, please be sure to register through the ASR member portal where you can access important information pertaining to your medical plans with ASR including participating providers, cost estimates for services, explanation of benefits/claims, and your accumulation amounts for deductibles and out-of-pocket costs. Looking for a Drug List? The drug list (or formulary) is reviewed throughout the plan year and sometimes changes are made. If you need to find out if a medication is covered by your health insurance plan, you should call EHIM’s Call Center at 800-311-3446.
Specialty Drugs are not covered under the plan. This level of medication is generally used to treat and manage complex conditions such as Multiple Sclerosis, Rheumatoid Arthritis and Oncology therapies. If your doctor is recommending a Specialty Drug for treatment, please call the EHIM Pharmacy Call Center, for assistance with alternative options at 800-311-3446.
Helpful Rx Cost Saving Tools & Tips!
MAIL ORDER - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you.
GOOD Rx - There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (insurance will not be applied and costs with the coupon will not count towards your deductibles and/or out-of-pocket maximums).
ASK YOUR DOCTOR – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.
The three medical plans all function the same when it comes to networks and access. All three plans provide coverage for both in and out of network services. However, if you do seek care out of network, you will experience higher cost sharing in addition to possible balance billing.

Health Savings Accounts (HSA)

The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible medical, dental, and vision expenses. The PPO HSA 3400 & the PPO HSA 6350 are qualified for an HSA. By contributing to an HSA, you reduce your taxable income, so you pay less in taxes — which saves you money.
HSA IRS Maximums are as follows and are on a calendar year basis. If you elect the HSA plan mid-year, maximums are prorated based on your effective date in the HSA plan.
$4,400 for a single contract | $8,750 for a two person or family
$1,000 "Catch Up" contribution for those age 55+

Dental

Patton Wings dental plan provides comprehensive coverage to help you and your family maintain good dental health. The dental plans include both in and out of network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.
If you are already enrolled the Patton Wings Delta Dental plan today and have not already done so, please be sure to register through the Delta Dental member portal where you can access important information pertaining to your dental plan with Delta Dental including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the Delta Dental portal as of your effective date of coverage.

The Delta Dental of Michigan plan offers in-network benefits when using a provider who participate in the Delta Dental PPO network. Coverage is also available for out-of-network services, however you will have higher cost sharing and may be balance billed.
When traveling out of the United States you also have access to the Passport Dental program giving you access to a worldwide network of dentists. English-speaking operators are available to help you access care. Delta Dental coverage outside of the United States is the same as Delta Dental coverage within the United States.

Vision

Patton Wings offers vision coverage through EyeMed to help pay for eye exams, prescription glasses and contact lenses. EyeMed also provides discounts for Hearing Care and Lasik.
If you are already enrolled in the Patton Wings EyeMed vision plan today and have not already done so, please be sure to register through the EyeMed member portal where you can access important information pertaining to your vision plan with EyeMed including participating providers, coverage information, and explanation of benefits/claims. Your vision network with EyeMed is the INSIGHT network. You may use the eye care professional of your choice. However, when you visit a participating in-network provider, you receive higher levels of coverage. If you choose to receive services out-of-network, you may be required to pay that provider at time of service and will need to submit a claim to EyeMed for the applicable reimbursement based on the out-of-network reimbursement schedule.

Financial Protection
Patton Wings understands the importance to you and your family to have strong financial protection. We provide all employees Basic Life/AD&D and Disability coverage at no cost to you. In addition to these benefits, we offer a variety of insurance you can choose from on a voluntary, employee paid basis so you can select what is best for your family. All of these coverages are provided to protect your income and out of pocket expenses.

A sudden accident or death can leave you or your loved ones in a vulnerable position. Patton Wings provides all eligible employees Basic Life and Accidental Death & Dismemberment (AD&D) coverage in the amount of $15,000 at no cost to you. Employees have the opportunity to enroll in additional Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Evidence of Insurability (EOI) is required for amounts in excess of the Guarantee Issue amounts, requests for increases of coverage during open enrollment and for those electing voluntary life for the first time after initially waiving the coverage.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
*Guarantee Issue only applies to new hires or newly eligible spouses (due to marriage) electing benefits for the first time.

Both Short & Long-Term Disability benefits are available to eligible employees.
Everyday illnesses or injuries can interfere with your ability to work, more serious issues can impact your ability to work indefinitely. Even a few weeks away from work can make it difficult to manage household costs. Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.

Patton Wings provides a competitive benefits package for all employees as part of our total compensation package. We realize you make benefit choices based on what’s important to you and the needs of your changing lifestyle. Therefore, along with our core benefits, we offer these additional benefits which help to provide financial support to you. These benefits are paid for through payroll deduction on a post-tax basis so that the benefits are tax free.
Accident, Critical Illness, & Hospital coverage is provided through Unum. Hopefully you are never in the position to use it, but if you were to experience an accident, illness, or hospitalization, it could end up costing you a lot of money. With these plans, you will be paid money for qualifying situations/services to cover our of pocket expenses or you may use the funds for whatever you like.
Accident: Concussion, Fracture, Internal Injury, Lacerations, Recovery Services, Prescription Drugs, Surgery, Treatments like X-ray, CAT scan, Pain Management Injections, and Emergency Transportation.
Critical Illness: Heart Attack, Stroke, Major Organ Failure, Coronary Artery Disease, Invasive Cancer, Skin Cancer, Dementia, MS, Parkinson's, Benign Brain Tumor, Occupational HIV, Hepatitis B, C, or D, and other Infectious Disease.
Hospital Indemnity: Hospital admission $500 is payable, and daily stay for up to 365 days $100 is payable.
BENEFIT PLAN PERK! Be Well Benefit
Your Unum plan pays a Be Well Benefit for one Be Well screening each year.
With Unum’s Be Well Benefit, you and other covered family members can receive a valuable incentive for important tests and screenings. Many of these tests are routinely performed, so it’s easy to take advantage of this benefit.
- Your Critical Illness Be Well benefit is $50.
- Your Accident Be Well benefit is $50.
- Your Hospital Be Well benefit is $50.

- Annual exams by a physician including sports physicals and well-child visits, dental and vision exams.
- Cancer screenings including pap smear, colonoscopy.
- Cardiovascular function screenings.
- Cholesterol and diabetes screenings.

- Immunizations including HPV, MMR, tetanus, influenza.
