We are pleased to offer a selection of benefits designed with the health and financial wellbeing of our diverse workforce in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for all employees and their families.

Employees
All full-time employees working 30 or more hours per week are eligible for the Padagis benefits package.

When To Enroll
New employees are eligible for benefits after 30 days of continuous, full-time employment. If you do not enroll during the initial eligibility period, the next time you are able to enroll is during annual Open Enrollment, unless you have a qualifying life event.
Qualifying Life Event
If you experience a Qualifying Life Event (QLE) during the year, you can make changes to your coverage. You must notify human resources within 30 days of the event date.

Padagis offers medical coverage through Blue Cross Blue Shield of Michigan. The plan provides freedom for you to use any qualified health care provider, but you receive the highest level of benefits when you use a provider who participates in-network. While the plan does not require you to have a primary care physician, e encourage you to have a doctor who is familiar with your medical history and needs.

CVS Pharmacy Resources
Prescription drugs are a major driver of health care costs. There are several ways to help you save.
Prescription drug benefits through CVS Caremark are included when you enroll in a Padagis Medical Plan, using the CVS Caremark Standard Formulary. Please see your benefits on the chart to the left.
You can purchase prescriptions through a national network of retail pharmacies or through the mail order Maintenance Choice program.
Maintenance Choice
You can save money by filling your maintenance prescriptions that you take regularly (such as high blood pressure, high cholesterol, and asthma) in 90-day supplies at CVS pharmacies or CVS Caremark Mail Service pharmacy. If you want to use this service but don't currently use the CVS pharmacy, you can fill your 34-day supply twice at your current pharmacy before being required to transfer your script.
Opt-out - If you do not want to use the Maintenance Choice program, you can opt-out by calling the customer care number on the back of your CVS ID card.
Save on Prescription Drugs
Review the standard formulary at www.caremark.com to determine where you drug falls. Please refer to specific plan # information which can be found on your ID card. We also encourage you to review all options with your physician and pharmacist.
Manage your Health Online
Review in-network providers and create your member profile by clicking the link above. You can also call 866.684.8213 for more information. Please have your ID card available when calling.
Through the carrier website, you can gain access to participating providers, health and wellness resources and forms. Personalized information is also available if you register with online Member Services. Once you register, you can check your current and past claims, view benefit information, request a benefit plan book or an ID card.

If you have enrolled in the CDH plan you may be eligible to open a Health Savings Account (HSA) through Optum. Health Savings Accounts are a great way to save money and pay for qualified medical expenses.
Great news! Padagis will continue to make contributions to employee's Health Savings Accounts. You must enroll in the CDH plan in order to receive the company contribution.
Please note: New hires will received a pro-rated contribution from Padagis depending on specific coverage start dates.

Funds roll over year to year?
YES. The money in the account belongs to you, even if you leave Padagis.
When are funds available?
The funds are available upon deposit from your paychecks. You do not have access to the money until it is in your account. You can use funds for eligible expenses now, or when you retire.
Padagis offers three (3) Flexible Spending Accounts (FSAs)
Administered by WEX Health
Healthcare FSA for employees not contributing to HSA
Limited Purpose FSA for employees on HDHP and contributing to HSA
Dependent Care FSA for employees with eligible dependent costs
Access the LEAP portal through WEX to use your account, submit reimbursements, view claim history and review fund balances.
Padagis offers a dental plan administered by Delta Dental of Michigan.
Find a Provider by going to www.deltadentalmi.com and search for a dentist in either the Delta PPO or Delta Premier neo. Search for dentist by name, clinic name, city or zip code, to find the provider that best suits your specific needs.
Padagis offers employees vision insurance through VSP. The plan provides specified dollar amounts or discounts for the purchase of eye glasses and contact lenses. Two plans are offered to you--a Basic Plan and Enhanced Plan. The Basic plan is offered to you free of charge. You pay to buy-up to the Enhanced plan offering (see rates below)

Life and accident insurance are valuable benefits for you and your family. Padagis automatically provides you with Basic Life and Accidental Death and Dismemberment (AD&D) 1.5 times your annual salary. Eligible employees are automatically enrolled in the company paid benefits when they become benefit eligible.

If you decline(d) coverage during your initial enrollment for yourself or your dependents, you may only enroll during the annual open enrollment period, and will be subject to Evidence of Insurability for any election amount.
Padagis provides full-time employees with short- and long-term disability income benefits– at no cost to you!
In the event you become disabled from a non-work-related injury or illness, disability income benefits will provide a partial replacement of lost income. Please note that you are not eligible to receive disability income benefits if you are receiving workers’ compensation benefits.
