Message from Human Resources

Helping you and your family reach and maintain good health is very important to Padagis. We are pleased to offer a selection of benefits designed with the health and financial wellbeing of our diverse workforce in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for all employees and their families.

Eligibility

Employees

All full-time employees working 30 or more hours per week are eligible for the Padagis benefits package.

Dependents

When you enroll yourself in the Benefit Program, you may also cover your eligible dependents for medical, dental, vision and supplemental life insurance coverage. Eligible dependents include:

  • Your legal spouse. This does not include a common law spouse or an ex-spouse.
  • Your (or your spouse’s) biological child, step child, legally adopted child, foster child, a child for whom legal guardianship has been awarded, and who are less than 26 years of age.
  • Your unmarried disabled dependent regardless of age. Proof of disability is required. Initial eligibility needs to be prior to age 26.

Enrollment

When To Enroll

New employees are eligible for benefits after 30 days of continuous, full-time employment. If you do not enroll during the initial eligibility period, the next time you are able to enroll is during annual Open Enrollment, unless you have a qualifying life event.

Qualifying Life Event

If you experience a Qualifying Life Event (QLE) during the year, you can make changes to your coverage. You must notify human resources within 30 days of the event date.

Common life events include:

  • Marriage or divorce
  • Birth or adoption of a child
  • Ineligibility of a dependent
  • Gain or loss of coverage by you or your spouse due to a job change
  • Employment status or transfer
  • Death of a spouse or dependent
  • Gaining eligibility for Medicare
  • Gaining or losing eligibility for Medicaid

How to Enroll

Enrollment will be done on the UKG (PlanSource) Portal.

  1. Log into your self-service account on UKG by clicking the below link.
  2. Review the job aid at the right for more instructions to access your enrollment module.
  3. Follow on-screen prompts through the demographic updating and election process.

IMPORTANT NOTE: At the end of your enrollment you will be taken to the Confirmation page. You must confirm your enrollment for it to save.

Medical Benefits

Provider: Blue Cross Blue Shield of Michigan

Padagis offers medical coverage through Blue Cross Blue Shield of Michigan. The plan provides freedom for you to use any qualified health care provider, but you receive the highest level of benefits when you use a provider who participates in-network. While the plan does not require you to have a primary care physician, e encourage you to have a doctor who is familiar with your medical history and needs.

Padagis employees have the choice of two (2) medical plans:

  • Option 1: $1,000-$25 PPO Copay Plan Plan - A traditional plan with office visit and prescription drug copays. The deductible for single coverage is $1,000 and family coverage is $2,000. After the deductible is met, the plan pays 80%. This option provides access to a Healthcare FSA.

  • Option 2: $2,000-80% CDH HSA Plan – A High Deductible Health Plan (HDHP), the deductible for single coverage is $2,000 and family coverage is $4,000. After the deductible is met, the plan pays 80%. This option provides access to a Health Savings Account (HSA).

Remember: Both plans have 100% preventive care coverage!

Health Savings Account (HSA)

Provider: Optum Bank

If you have enrolled in the CDH plan you may be eligible to open a Health Savings Account (HSA) through Optum. Health Savings Accounts are a great way to save money and pay for qualified medical expenses.

Great news! Padagis will continue to make contributions to employee's Health Savings Accounts. You must enroll in the CDH plan in order to receive the company contribution.

Please note: New hires will received a pro-rated contribution from Padagis depending on specific coverage start dates.

Eligible employees:

To be eligible to open and contribute to an HSA:

  • You must have coverage under a qualified HDHP
  • Participants cannot be covered by any other health insurance plan (does not apply to certain types of insurance, such as dental, vision, disability or long-term care coverage)
  • Participants cannot participate in Healthcare FSA or spouse/domestic partner’s Healthcare FSA or Health Reimbursement Account (HRA)
  • You cannot be eligible to be claimed as a dependent on someone else’s tax return
  • Participants cannot be enrolled in Medicare or Medicaid
  • You have not received Department of Veterans Affairs Medical benefits in the past 90 days unless the Veteran has a disability rating (There may be additional special circumstances, check with your tax preparer)

Money you contribute:

2025 Limits: No minimum required: $4,300 maximum (individual), $8,550 maximum (family), $1,000 catch-up (anyone over age 55)

Tax advantage

  • Pay no taxes on the money you contribute
  • Pay no taxes on interest you earn
  • Pay no taxes when you withdraw money -- Triple-tax savings for you!

Eligible expenses

Eligible healthcare expenses for you and your enrolled dependents now or in the future (retirement). Visit www.IRS.gov and review Publication 502 for a list of eligible expenses

How to use the account

Use your HSA Card or pay out-of-pocket and submit your claim for reimbursement. Visit www.optumbank.com for more information!

Funds roll over year to year?

YES. The money in the account belongs to you, even if you leave Padagis.

When are funds available?

The funds are available upon deposit from your paychecks. You do not have access to the money until it is in your account. You can use funds for eligible expenses now, or when you retire.

HSA Limits

Flexible Spending Account(s)

Padagis offers three (3) Flexible Spending Accounts (FSAs)

Administered by WEX Health

Healthcare FSA for employees not contributing to HSA

  • Up to $3,300 for medical, dental, vision or hearing expenses

Limited Purpose FSA for employees on HDHP and contributing to HSA

  • Up to $3,300 for dental, vision or post-deductible medical expenses ONLY

Dependent Care FSA for employees with eligible dependent costs

  • Up to $5,000 for eligible dependent care expenses (children under the age of 13, disabled dependents, elder care)
  • Maximum is $2,500 if married filing separate tax returns. 

Access the LEAP portal through WEX to use your account, submit reimbursements, view claim history and review fund balances.

Dental

Provider: Delta Dental of Michigan

Padagis offers a dental plan administered by Delta Dental of Michigan.

Find a Provider by going to www.deltadentalmi.com and search for a dentist in either the Delta PPO or Delta Premier neo. Search for dentist by name, clinic name, city or zip code, to find the provider that best suits your specific needs.

Vision

Provider: VSP

Padagis offers employees vision insurance through VSP. The plan provides specified dollar amounts or discounts for the purchase of eye glasses and contact lenses. Two plans are offered to you--a Basic Plan and Enhanced Plan. The Basic plan is offered to you free of charge. You pay to buy-up to the Enhanced plan offering (see rates below)

Life and AD&D Insurance

Provider: Reliance Matrix

Life and accident insurance are valuable benefits for you and your family. Padagis automatically provides you with Basic Life and Accidental Death and Dismemberment (AD&D) 1.5 times your annual salary. Eligible employees are automatically enrolled in the company paid benefits when they become benefit eligible.

Voluntary Life and AD&D

In addition to the Basic Life and AD&D coverage, you can also enroll in additional coverage paid for by you.

Employee:

  • Benefit Amount: 1x, 2x, 3x, 4x or 5x annual earnings up to $1,000,000
  • Guarantee Issue Amount (when first eligible): $500,000 ($250,000 if age 70 or older)

Spouse:

  • Benefit Amount: Increments of $10,000 up to $100,000, not to exceed 100% of employee's election
  • Guarantee Issue Amount: $50,000 ($25,000 if age 70 or older)

Children:

  • Benefit Amount: Birth up to 15 days: $1,000, Children 6 months – 26 years old: up to $25,000 in increments of $5,000
  • Guarantee Issue Amount: Full benefit

Open Enrollment Rules:

If you decline(d) coverage during your initial enrollment for yourself or your dependents, you may only enroll during the annual open enrollment period, and will be subject to Evidence of Insurability for any election amount.

Disability Insurance

Provider Reliance Matrix

Padagis provides full-time employees with short- and long-term disability income benefits– at no cost to you!

In the event you become disabled from a non-work-related injury or illness, disability income benefits will provide a partial replacement of lost income. Please note that you are not eligible to receive disability income benefits if you are receiving workers’ compensation benefits.

Short Term Disability (STD): Income replacement during a temporary disability or illness

  • Benefits are available up to six (6) months depending on years of service for an approved disability. Please contact Human Resources for additional information pertaining to specific eligibility.

Long Term Disability (LTD): Income replacement during an extended disability or illness

  • Benefit Percentage: 60% of your covered monthly earnings up to $10,000 per month
  • Benefit Waiting Period: 180 days (typically satisfied while collecting a Short Term Disability benefit.
  • Maximum Benefit: SSNRA- Social Security Normal Retirement Age
  • Pre-Existing Condition Applies - 3/12
    • The policy has a 3/12 pre-existing conditions limitation provision, which excludes coverage for anything you have been treated for 3 months prior to coverage until you have been on the policy and treatment free for 12 consecutive months. Please see the benefit summary for full details of this provision.

Legal Plan

Provider: MetLife Legal Plans

We are pleased to introduce the new voluntary Legal Plan offered through MetLife!

The cost of this plan is only $18 per month and covers you and your immediate family!

Legal issues occur throughout life, when employees are getting married, buying a home, becoming a caregiver or handling financial matters like debt or tax audits. Dealing with these matters can be costly and time consuming, taking employees away from work and impacting their overall well-being.

MetLife provides your employees with the cost-effective, multi-channel access to legal help they need to easily handle costly legal matters in their life—helping them to feel more financially and emotionally secure.

You are allowed to choose an attorney from the MetLife network, or outside of it, or use their digital tools to handle matters. With a large network of attorneys and the ability to complete estate planning or download self-help documents on their website, you have the flexibility to choose how you want to handle your legal matter.

401(k)

Provider: Empower

One of the best ways to start saving for the future is through the Padagis 401(k) retirement plan!

How does it work?

  • For every $1 you contribute of the first 2% of your pay, Padagis will contribute $1. Then, for every $1 you contribute of the next 2% of your pay, they’ll contribute $0.50. So to get the full amount, you’ll need to contribute at least 4% of your pay.
  • Padagis will also make a profit sharing contribution to your 401(k) account equal to 3% of your pay. This contribution will be made in a lump sum every April.
  • Together, the full company match and profit sharing contribution equal 6% of your salary per year! And you are immediately vested in all contributions to your account.

How much can I contribute?

  • You can contribute up to 50% of your pay on a pre-tax, Roth 401(k) after-tax, or traditional after-tax basis, or a combination of the three. The IRS also sets dollar limits on contributions. The annual contribution limit is $23,500 for 2025 with employees age 50+ able to contribute "catch-up" dollars of up to $7,500.

When can I join the plan?

  • If you meet service requirements, you are eligible to start making contributions immediately after the completion of one month of service. If you do nothing, you’ll be automatically enrolled 30 days after your hire date.
  • You’ll be automatically enrolled in the plan at a pre-tax contribution rate of 4% of your pay. Your pre-tax contribution rate will increase automatically by 1% every September, unless you choose a different month. You can also change this rate, up to a maximum of 3%. Your contributions will be automatically invested in the EmpowerTarget Retirement Fund with the target date closest to the year in which you will reach age 65.
    • NOTE: Investments in Target Retirement Funds are subject to the risks of their underlying funds. The year in the fund name refers to the approximate year (the target date) when an investor in the fund would retire and leave the workforce. The fund will gradually shift its emphasis from more aggressive investments to more conservative ones based on its target date. The Income Fund has a fixed investment allocation and is designed for investors who are already retired. An investment in a Target Retirement Fund is not guaranteed at any time, including on or after the target date.
