Annual Open Enrollment

Annual enrollment is your opportunity to learn about your health care benefits, review your current coverage and choose the best options for you and your family.

Your Medical, Medical Gap and Vision plan year runs from December 1, 2021 through November 30, 2022.

Your Dental plan year runs from October 1, 2021 through September 30, 2022

Your Long-Term Disability plan runs from April 1, 2022 through March 31, 2023

Please review all 2021-2022 Benefits Overview Information. If no changes are needed then your current benefits will just roll over into the new plan year.


If you are a new employee you have the opportunity to elect coverages.

Please make your elections no later than 30 days from your hire date.

Coverage will begin the 1st of the month following your hire date.

How To Enroll

Enrollment Forms

Current Employees:

  • If you'd like to change your existing coverage or make new elections, you must do so within the 30 day window prior to the renewal date of coverage.
  • Complete the appropriate application/change form and give it to Danielle Boston.
  • Your current medical, medical gap, dental and vision elections will carry forward to 2021-2022 if you don't make a new election.
  • Current employees are already enrolled in the long-term disability insurance and do not need to do anything to renew it.


New Hires:

You will need to complete the medical, medical gap, dental, vision and long-term disability applications.

Please give the applications to Danielle Boston no later than 30 from your hire date.

If you are a NEW HIRE and need to enroll for the FIRST time please see above section How to Enroll.

All enrollment forms are attached for download.

If you enroll in the Highmark medical you will automatically be enrolled in the Companion Life Medical Gap.
