There are two parts to enrolling:
All employees should complete their election through Employee Navigator by Friday, 4/22/2022.
STEP 1: Log In
First time users: Click on your registration link in the email sent to you by your admin or "Register as a new user". Create an account, and create your username and password.

STEP 2: Welcome!
After you login click "Let's Begin" to complete your required tasks
STEP 3: Onboarding (for first time users, if applicable)
Complete any assigned onboarding tasks before enrolling in your benefits. Once you've complete your tasks click "Start Enrollment" to being your enrollments
STEP 4: Start Enrollments
After clicking "Start Enrollment", you'll need to complete some personal and dependent information before moving to your benefit elections
STEP 5: Benefit Elections
To enroll dependents in a benefit, click the checkbox next tot he dependent's name under "Who am I enrolling?"
Below your dependents you can view your available plans and the cost per pay. To elect a benefit, click "Select Plan" underneath the plan cost
Click "Save & Continue at the bottom of each screen to save your elections
If you do not want a benefit, click "Don't want this benefit?" at the bottom of the screen and select a reason from the drop-down menu
STEP 6: Review and Confirm Elections
Review the benefits you selected on the enrollment summary page to make sure they are correct then click "Sign & Agree" to complete your enrollment. You can either print a summary of your elections for your records or login at any point during the year to view your summary online

Anyone enrolling with Sedera must complete their online enrollment with Sedera through the new portal by Friday, April 22nd, to ensure they will have a May 1st effective date. Otherwise, your membership may be pushed to the next month.

This is the part of your combo that covers your preventative care at 100%. You have two options to choose from. Both options cover your preventative care at 100%
OPTION 1: HSA MEC
This HSA compatible health plan provides the framework for an HSA plan.
OPTION 2: Ultimate MEC
This plan offers copays for office visits as well as diagnostic x-ray, laboratory services, and a long list of medications.
You will receive an ID card for this plan.
PHCS network search instructions:
●Go to www.multiplan.com
●Click on Find a Provider in the upper right corner
●Click on the green Select Network button & choose PHCS
●Click the green Select Network box
●Choose PHCS in the pop-up box
●Select Preventive Services Only
Services billed by a hospital are not covered on these plans.

You may select the dentist of your choice; however, when you visit a participating in-network dentist you could have lower out-of-pocket costs, no balance billing, and claims will be submitted by your dentist on your behalf.
This plan uses the Platinum (Co-Insurance) network.
