About This Guide
This Benefits Guide describes the highlights of the Prison Fellowship Benefits Programs in non-technical language. Your specific rights to benefits under this program are governed solely, and in every respect, by the official plan documents and not the information contained within this Benefits Guide.
In the event there is any discrepancy between the descriptions of the program elements contained within this Benefits Guide and the official plan documents, the language of the official plan documents shall prevail. Please refer to the plan-specific documents published by each of the respective carriers or third-party administrators for detailed plan information. Eligibility for any benefit plan is determined by plan documents and policies. You should be aware that any and all elements of our Benefits Program may be modified in the future to meet Internal Revenue Service rules, or otherwise as determined by Prison Fellowship.

- Employee Plus Child(ren)
- Family Coverage
Coverage begins on the first day of the month following your date of hire.

Prison Fellowship offers you a credit if you choose to waive medical coverage with PF.

This amount is paid out on a bi-weekly basis.
*Proof of other coverage may consist of an ID card reflecting all family members covered, certificate of coverage reflecting family members covered, or a printout reflecting coverage from your current carrier portal.
Benefits
This summary is intended as a brief highlight of high encounter benefits and should not be relied upon to fully determine coverage. The Booklet/Certificate governs and includes details not compared above.
Prescription Drugs
GENERIC VS. BRAND FORMULARY VS. BRAND NON-FORMULARY
You can control your out-of-pocket cost based on the medications your physician prescribes. To get the greatest value from your prescription benefits, ask your doctor to prescribe generic medications whenever possible.
MAIL ORDER & MAINTENANCE MEDICATIONS
Use Mail Order for maintenance medications that you routinely fill each month. You may be able to obtain valuable savings on 90-day supplies of medications.
NOTES REGARDING THE PRESCRIPTION DRUG COVERAGE
The Rx plan does have a mandatory generic feature that requires you to take a generic drug. If you take a brand name drug when a generic is available, you will be responsible for the brand name copay and the cost difference of the drug between the generic and brand cost. If your doctor requires that you take the brand name drug, the doctor may write "DAW" (dispense as written) on your prescription and you will not be charged the difference in cost between the generic and brand drug. The plan requires step therapy meaning that you may be required to try a less expensive alternative drug that has been proven to treat the same ailment before the plan will cover the higher cost drug. If you have tried the less expensive drug and the past and it was not effective for you, your doctor may write a letter to describe this and the plan will waive the step therapy mandate and you may continue to take the same higher cost alternative that you were prior.
Specialty injectable drugs do require prior authorization.
Be sure to check the drug Formulary list for your medication. If you are taking a Non-Formulary drug, you may find an equivalent drug on the Formulary list which would save you additional dollars with each fill. If your drug falls on the Formulary list, it would fall under Tier 2, which has a cheaper copay then Tier 3. Please note that the Formulary list does change throughout the year. Please be sure to check your drugs on the UMR website to determine if they fall on the formulary list. If you find that your drug has been excluded at any time, please be assured that there are other equivalents that are covered. If you have tried these other medications and they were non-effective for you, you may work with your provider to submit an appeal to UMR for review.
The Rx plan excludes clinical duplicates, meaning the plan will exclude medications with similar chemical composition to existing medications. Also, the Copay Card Accumulator Adjustment (CCAA) program will be added to the plan. This means that if you are receiving a manufacturer coupon towards a high cost specialty drug, only the amount that you physically pay out of pocket will be accumulated towards your plan out of pocket maximum.

Flexible Spending Account (FSA)
Prison Fellowship offers a Section 125 Flexible Spending Account (FSA) through American Benefits Group, which allows you to pay for qualified healthcare and dependent care expenses. You are eligible for this plan on the first of the month following your date of hire.
The money that goes into an FSA is deducted from your bi-weekly pay on a pre-tax basis (before Federal and Social Security taxes are calculated). Because you do not pay these taxes on money that goes into an FSA, you decrease your taxable income and potentially increase your spendable income.
Health Care FSA
A Health Care FSA provides you with the ability to save money on a pre-tax basis for any IRS-allowed health expenses not covered by your medical benefits plan. These expenses include deductibles, copays and coinsurance payments, non-reimbursed medical, dental and vision expenses, qualified over-the-counter product costs, Rx copays, hearing care expenses, and orthodontia. The maximum annual amount you can contribute to a Health Care FSA is $3,400.
How much money you should put into an FSA each pay period depends on your expenses. The best way to estimate your expenses for the year is by looking over the eligible expenses you incurred over the past few years and then divide the total predictable expenses by the number of pay periods in the year.
Dependent Care FSA
A Dependent Care FSA provides you with the ability to set aside money on a pre-tax basis for day care expenses for your eligible dependent(s). Generally, expenses will qualify for reimbursement if they are the result of care for:
- Your children, under the age of 13, for whom you are entitled to a personal exemption on your federal income tax return; and/or
- Your spouse or other dependents, including parents, who are physically or mentally incapable of self care.
Please Note: This benefit may only be used to pay for dependent care services that enable both you and your spouse to work full-time, seek employment and/or attend school. This does not include overnight camp or overnight care. The maximum annual amount you can contribute to a Dependent Care FSA is $7,500.
Important FSA Features
It is very important that you only elect to contribute an amount to the FSA that you can reasonably expect to spend in 2026. Before you elect to contribute to either a Health Care or Dependent Care FSA, please note the following important features of these plans.
- The amount you elect may not be changed during the plan year unless you experience a qualifying life event.
- You may not transfer funds between a Health Care and a Dependent Care FSA.
- FSA funds do not roll over at the end of the plan year. This is a "use-it or lose-it" policy.
- Your participation in an FSA will end on the date of your termination of employment. You have 60 days following your coverage end date to file a claim for reimbursement of expenses (i.e. expenses incurred on or before the date of your employment termination).
Example Savings
These examples have been designed to help you understand the value of your Flexible Spending Accounts. They are intended as sample illustrations only.
EXAMPLE 1:
This example is based on the individual salary of Mary Jane Doe. She is single, earns $28,500 a year, and claims the standard deduction. She contributes $2,000 to her Health FSA account. It does not take into account any potential savings for contributions toward her insurance premiums or retirement accounts.
EXAMPLE 2:
John Doe and his spouse earn $42,500 a year, file a joint return, and claim the standard deduction. They have on 10 year old child and contribute $5,000 to their Dependent Care Assistance Plan. This example does not take into account any potential savings for contributions towards their insurance premiums or retirement accounts.

Supplementary Benefits
*Note: Prison Fellowship is not endorsing these benefits or encouraging participation. Our goal is to educate and make available these options. It covers accidents that occur on and off the job, with a range of incidents from common injuries to more serious events. Some of those are: fractures, burns, concussions, dental extractions, eye injuries, ambulance rides, emergency room treatment, and hospital confinement.
The policy also pays you a flat amount per day if you are hospitalized for an accident and includes an accidental death benefit. There is a wellness benefit built into the plan that will earn you $50 per calendar year for obtaining your wellness visit with your doctor. The plan is fully funded by you.
Voluntary Critical Illness
Unum's Voluntary Critical Illness plan is designed to help you with the out-of-pocket expenses and lost wages associated with serious illnesses. You may select coverage for yourself only or yourself and your family. You may select up to a $30,000 benefit for yourself and up to $30,000 for your spouse (ages 17 to 64). The same benefit amount will be provided for your spouse and child if you elect family coverage. This benefit pays out a lumpsum benefit, tax-free, if you are diagnosed with one of the covered illnesses. Some of the illnesses include: heart attack, stroke, major organ failure, blindness, and cancer. The plan is fully funded by you and based on your own demographics.
There is a wellness benefit built into the plan that will earn you between $50 and $100 (depending on coverage amount selected), per calendar year for obtaining your wellness visit with your doctor.
Voluntary Hospital Indemnity

ALLOWED AMOUNT
The insurance carrier’s predetermined dollar amount for a specific service, based on reasonable and customary charges.
BALANCE BILLING
An out-of- network healthcare provider billing a patient for the difference between what the patient’s health insurance reimbursement amount is and what the provider charges.
Explanation:
The most important advantage of having health insurance is the ability to benefit from cost negotiations between the insurance company and the provider. When seeing an in-network provider, the patient pays an amount that has previously been negotiated and discounted with the insurance company (via a provider contract). These savings can result in hundreds to thousands of dollars. When seeing a provider that does not participate with the insurance company (aka out-of-network), there is no contract between the insurance company and the provider. This means that the amount the provider is permitted to bill the patient has not been negotiated or discounted in any way. If the patient's plan has out-of-network benefits, the insurance company will still pay a portion of the out-of-network claim, but their payment is based on the amount they would pay an in-network provider (known as the "allowed amount"), not the provider’s total billed amount. Since there is no contract in place, the provider is not required to write off the cost difference between what the insurance company pays and what the provider charges and can bill the patient the difference. This is balance billing.
Here is an example: Monica has knee surgery and decides to use a surgeon that is not participating with the insurance company. Monica has insurance coverage through her employer, and the plan pays 80% of the allowed amount. Monica's share will be 20% according to the benefit plan.
- Provider bills $2,800 for the service
- The Insurance company's allowed amount for this service is $1,800
- The insurance company pays the claim according to the benefits on Monica's plan which is 80% of the allowed amount of $1,800 = $1,440. Monica is responsible for 20% of $1,800 = $360.
- Because Monica chose to see an out of network provider, the provider can balance bill her the difference between their total billed amount for the surgery and the insurance company’s allowed amount ($2,800 - $1,800 = $1,000 in balanced billing charges)
In this example, Monica will owe the provider the balanced billed amount as well as the 20% coinsurance from the benefit plan, which is a total of $1,360. If Monica had seen a provider that was in-network with the insurance company, she would only owe $360. An in-network provider is required by contract to disregard the difference between the amount billed and the amount allowed.
COINSURANCE
The percentage of costs of a covered health care service you pay after you’ve paid your deductible. For example, if you pay 5 percent of an in-network covered charge, the plan pays 95 percent.
Important Note: Coinsurance percentages are based on the insurance carrier’s allowed amount, not the provider’s total amount billed
CO-PAYMENT
A fixed amount you pay for a covered health care service after you’ve paid your deductible.
DEDUCTIBLE
The amount you pay for covered health care services before your insurance plan starts to pay.
Embedded deductible means that any member of a family will not have to pay more than the individual deductible before he/she begins to pay coinsurance.
Non-embedded deductible means that the entire family deductible must be satisfied before any member of the family begins to pay coinsurance.
EMERGENCY SERVICES
Evaluation of an emergency medical condition and treatment to keep the condition from getting worse.
FORMULARY
A list of prescription drugs that are covered by your health insurance plan. The formulary is separated into cost levels called tiers, which affects how much you pay for each drug. Also known as a Prescription Drug List (PDL).
HEALTH SAVINGS ACCOUNT | HSA
A tax advantaged savings account that is owned by you and can be funded with tax free contributions for the purpose of paying for qualified health care expenses today and in the future. HSA’s can only be funded by those who are enrolled in a Qualified High Deductible Health Plan (HDHP).
NON-PREFERRED PROVIDER
A provider who doesn’t have a contract with your health insurer or plan to provide services to you; also known as an out-of-network provider. You’ll pay more to see a non-preferred provider. Check your policy to see if you can go to all providers who have contracted with your health insurance or plan, or if your health insurance has a tiered network and you must pay extra to see some providers.
OUT-OF-POCKET MAXIMUM
The most you could pay during a plan year for your share of the costs of covered services. After you meet this limit, the plan will pay 100% of the allowed amount. There are separate in- and out-of-network out-of-pocket maximums. All copays, deductibles, and coinsurance accrue to the out-of-pocket maximums. Your out-of-pocket maximum is on a contract year basis. Refer to your healthcare plan summaries for more information.
PRE-AUTHORIZATION
A decision by your health insurer or plan that a health care service, treatment plan, prescription drug is medically necessary.
PREMIUM
The amount that must be paid for your health insurance or plan each month. This amount is shared by you and your employer.
PRIMARY CARE PHYSICIAN
A physician who directly provides or coordinates a range of health care services for a patient.
SPECIALIST
A physician specialist focuses on a specific area of medicine or a group of patients to diagnose, manage, prevent, or treat certain types of symptoms and conditions.
UCR (USUAL, CUSTOMARY AND REASONABLE)
The amount paid for a medical service in a geographic area based on what providers in the area usually charge for the same or similar medical services. The UCR amount sometimes is used to determine the allowed amount.
URGENT CARE
Care for an illness, injury or condition serious enough that a reasonable person would seek care right away, but not so severe as to require emergency room care.
