You are eligible to participate if you are full-time and work a minimum of 30 hours per week. Your coverage will be effective 1st of the month following 30 days from your date of hire.

ENROLLMENT INSTRUCTIONS:

1.You will receive an Email with a link to Ease. Click the blue “login” button with the email message to be directed to the enrollment portal.

2.Enter your User ID and password

~ If you do not remember your User ID and Password please contact Kelli Krise with NFP at kelli.krise@nfp.com.

3.Follow instructions and enroll in your benefits

4.Make sure to sign your forms, save your elections and print your confirmation statement.

Here’s how accident insurance works

Accident insurance can help you and your family manage out-of-pocket expenses that result from an

accidental injury. The coverage pays tax-free cash benefits directly to you, regardless of other insurance

coverages or actual expenses … and the benefits can be used however you need them. That could mean

anything from medical expenses like emergency care, exams, and hospital stays to other expenses,

including transportation, food, and lodging. You’re in control.


Who can buy coverage?

You may buy coverage if you’re an active, full-time employee working at least 30 hours a week. Seasonal,

temporary, or contract employees can’t purchase.

o If you’re on a regularly scheduled day off, holiday, vacation day, jury duty, funeral leave, or personal

time off, you’re still considered actively at work, as long as you’re fulfilling your regular duties and were

working the day immediately prior to your time off.

o You must enroll within 31 days of being eligible. If you don’t, you’ll have to wait until the next open

enrollment period.

If you’re covered, you may buy coverage for your dependents, if they’re not confined at home, in a

hospital or skilled nursing facility (this is referred to as Period of Limited Activity).
