BCBSMN administers your medical and prescription drug benefits this year. The medical plan includes a prescription drug benefit, which covers prescriptions at participating pharmacies and mail-order maintenance drugs.

MP Nexlevel's dental plan provides comprehensive coverage to help you and your family maintain good dental health.

MP Nexlevel offers vision coverage through MetLife to help pay for eye exams, prescription glasses and contact lenses. In addition the plan provides benefits for prescription safety glasses. Review your coverage details in your Benefit Guide.

MP Nexlevel provides you with Basic Life and AD&D coverage. You have the option to enroll in Voluntary Life/AD&D coverage through MetLife.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Voluntary Short Term Disability
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. You have the option to enroll in Short Term Disability coverage through MetLife. Benefits begin after a 14 day waiting period. The benefit pays 60% of your income to a maximum of $2,500 per week.
Voluntary Long Term Disability
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. You have the option to enroll in Long Term Disability coverage through MetLife. Benefits begin after a 90 day elimination period. Benefits are equal to 60% of covered monthly earnings up to the stated maximum based on your employment classification.

We continue to offer health care and dependent care flexible spending accounts (FSAs) for eligible employees*, administered by Alerus, which allow you to pay for eligible health and dependent care expenses with pretax dollars. The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.
*Eligible employees are those not subject to seasonal layoff.
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.

How the Plan Works
You must incur your eligible expenses during the plan year — January 1 to December 31. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service. You do, however, have until March 31st annually to file your claims. Any funds after March 31 unclaimed will be forfeited.

Building a strong financial foundation for your future is an ongoing process. MP Nexlevel's 401(k) Plan, managed by Fidelity, is designed to help you save for retirement by offering tax advantages as well as the convenience of payroll deductions.
Eligibility and Enrollment
You become eligible to participate in the 401(k) Plan if you worked at least 6 months are at least 21 years of age. You will be automatically enrolled into the 401(k) plan once you become eligible unless you elect not participate in the Plan by declining your enrollment.

For 2024, BlueCross BlueShield is rolling out a new member app called Blue Care Advisor. This will be replacing the BCBMN Member App and Sharecare. Blue Care Advisor is tool to help you connect to all your benefits, claims, programs, rewards and more. If you currently have a login for the BCBSMN Member app, this will be the same login for Blue Care Advisor. You will just need to download the Blue Care Advisor app or login with the link below. If you do not yet have a login for BlueCross BlueShield, you can register with the link below.
