Summary of Benefit Changes Effective July 1, 2024

Medical Changes - Buy-Up EPO Plan

• The Co-Insurance will be increasing from 20% to 30%

Medical Changes - Core EPO Plan

• The deductible will be increasing from $2,500 / $5,000 to $4,000 / $8,000
• The out-of-pocket maximum will be increasing from $6,350 / $12,700 to $8,550 / $17,100
• Members accessing care out of the tri-state area will have the Core Network instead of the Choice Plus Network. Please review the Liberty Network Change flyer in the Additional Resources section for more detailed information
• For help locating Oxford Liberty Network providers, members should sign in to myuhc.com and use the provider search feature or access it from the UnitedHealthcare app
