New Hire Enrollment

You must actively enroll within 30 days after you have satisfied your new hire waiting period which is first of the month following date of hire. This is your chance to review your personal situation and consider your Proclinical plan choices. Outside of this enrollment period, you may only make benefit changes during the annual Open Enrollment period, or if you experience a qualified life event (e.g., birth, adoption, marriage, divorce, loss of spouse/partner health coverage). Elections can be made online via the Paychex benefits portal.

United Healthcare (UHC) is the carrier for the Medical, Dental, Vision and Basic Life Plans


UHC Medical


  • Choice Plus Network
  • Three plans to choose from (1 PPO Plan and 2 High Deductible Health Plan (HDHP))
  • 3-Tier Prescription Drug Coverage
  • Prescription Drug Coverage and Preferred Drug List -with a large national network of retail chains and independent pharmacies, home delivery and online tools to help you compare and save
  • Health and Wellness Resources - online and telephonic resources to help you get and stay healthy
  • Customer Service - you can contact UHC by phone at 866-414-1959, online at www.myuhc.com, or via Facebook and Twitter


How To Search for an In-Network Provider


  1. Go to www.myuhc.com
  2. Click on "Find a Provider"
  3. Select the type of provider
  4. Click on "Employer and Individual Plans"
  5. Click on "Shopping Around"
  6. Click on "Choice Plus Network"
  7. You can then search by provider name or type of service


UHC Dental


  • National Options PPO 20 Network
  • Annual Preventive Service covered 100%, both in and out of network, not subject to the annual deductible


UHC Vision


  • UHC Vision Network - large national network to take advantage of personalized eyecare at a private practice or convenient favorite retail chain
  • To find a provider, sign in to www.myuhcvision.com or call 800-638-3120


UHC Basic Life


  • The Basic Life Insurance is automatically provided by Proclinical Staffing
  • Coverage includes Basic Life and Basic Accidental Death and Dismemberment

How To Enroll

It starts with Paychex
  • At the start of your New Hire Open Enrollment, you will receive a welcome email from Human Resources with your Paychex registration instructions.
  • You will have to select “Health & Benefits” on the left-hand side of their screen.
  • All elections must be made within 30 days of your date of hire
  • HDHP HSA Plan Members, decide if you want to make a contribution to your Health Savings Account
  • Please be sure to review all the benefits offered, and either elect or waive each line of coverage. The elections you make cannot be changed once the New Hire Open Enrollment period closes unless you have a qualifying life event.
