All full-time employees working an average of at least 25 hours per week are eligible to participate in the PHG benefits program. You must be actively at work for your coverage to be effective on your eligibility date. Beginning on the first day of the month after 60 days from your date of employment, you will be eligible for the PHG Benefit Programs. You will receive an invitation via Paylocity to enroll in benefits, you will have 15 days to login to make your benefit selections. A reminder email will be sent, but if you chose not to elect benefits during this enrollment window, you will be only be eligible to enroll for the 2022 Benefit Plan Year during our open enrollment period to begin in November 2022.
During the benefit selections process, if you elect the HSA Health Insurance Plan and decide to contribute funds to a Health Savings Account, you will need to setup an account and provide the bank account and routing number to firstname.lastname@example.org no later than the insurance effective date. This will ensure the account information is setup on your pay profile prior to the first contribution deduction, which will occur on the first payroll of the month the insurance is effective. We have provided information for HSA Authority, the banking institution that we have an established relationship with. If you currently have an HSA account with another institution, you can also choose to deposit your Biweekly HSA contributions to that account.

You will be eligible to participate in the organization's 401k plan beginning on the first day of the month after 60 days from your date of employment. We will auto enroll you with a contribution of 5%. Please use the 401k enrollment guide to setup your online Nationwide Account. If you wish to contribute a different %, please go online to Nationwide to update that amount. The company will match 25% of the first 5% you contribute.
