Welcome to New Hope Academy's benefit landing page, here you will find summaries of all the benefits available to you as an employee of New Hope Academy. To enroll in your benefits you will log into the Benefit Admin system Ease. You will be receiving and email from the Ease system prompting you to register for the system and elect your benefits. If you do not receive the email, please check your spam. If it is not in your spam please reach out to Dave.
The "2022 Benefit Guide" will provide a brief overview of the benefits offered by NHA. To learn more about each benefit, click on the sections below.
To enroll in your benefits you will receive an email from Ease. Even if you plan on waiving the benefits, please log in and go through the enrollment process. There are benefits that you are automatically enrolled in and we want to make sure you know what is available to you.
NHA offers two medical plans by BlueCross BlueShield of Illinois: PPO & HMO.
Dental coverage is provided by Guardian.
Vision coverage is provided through Unum.
The life, short & long-term disability are covered at 100% by NHA.
This is voluntary life insurance which is paid by the employee.
Employee and Spouse: Choose from a minimum of $10,000 to a
maximum of $500,000 (in $10,000 increments) for yourself and/or
your spouse. The benefit amounts chosen need not be the same.
Eligible Dependent Child(ren): 14 Days to 6 months: $1,000
Age 6 months to 20 years of age (26, if full-time student): choice of
$2,500, $5,000; $7,500 or $10,000
Choose one benefit amount for all eligible children in family.
GUARANTEED ISSUE (INITIAL ELIGIBILITY PERIOD ONLY)
Under age 70: $50,000
Age 70 and over: none
Under age 60: $10,000
Age 60 or older: none
NHA offers a 401K that you are eligible for after 6 months of full time employment.
