We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

ELIGIBILITY AND ENROLLMENT
Eligibility
All full-time employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Michigan MultiKing. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.
Children are eligible for benefits as follows:
- Medical and Vision: Until the last day of the year in which the child turns 26
- Dental: Until the date the child turns 20, or the date the child turns 26 if a full-time student
Enrollment
ENROLLMENT INSTRUCTIONS:

- You must complete and sign an enrollment form, even if you are waiving coverage.
The Benefit choices you make will remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

MEDICAL/Rx
Michigan MultiKing offers two medical and prescription drug options: two through Blue Care Network (BCN). The 2023 options are:
- BCN Routine Care HMO 5000
- BCN HMO HSA 6350

Both Blue Care Network options are HMOs, meaning that, other than for emergency care, services are covered only if you see a provider that is in-network. You must select a primary care physician (PCP), and you must obtain a referral from your PCP for other care.

VIRTUAL VISITS
BCN allows you to see a provider virtually from your mobile device or your computer. You can use virtual visits when you have a non-emergency condition and:
- your doctor is not available;
- you become ill while traveling; or
- you are considering visiting a hospital emergency room for a non-emergency health condition.
Some examples of non-emergency conditions that can be treated through virtual care:
- bladder infection
- bronchitis
- fever
- rash
- flu
- sore throat
- stomach ache
